经阴道彩色多普勒超声对输卵管间质部妊娠的诊断分析

Value of Transvaginal Color Doppler Ultrasound in Diagnosis of Interstitial Tubal Pregnancy

目的探讨经阴道彩色多普勒超声(简称超声)对输卵管间质部(简称间质部)妊娠的定位及定性诊断价值。方法对28例经手术病理结果证实的间质部妊娠声像图特征进行分析。结果 28例间质部妊娠病例,超声诊断正确率75.0%(21/28)。根据声像图表现分为三型:即妊娠囊型、包块型、破裂型。妊娠囊型9例,1例误诊为宫角妊娠(8/9);包块型16例,1例误诊为宫角妊娠,2例误诊为滋养细胞疾病(3/16);破裂型3例,均诊断为宫外孕破裂出血,未明确定位诊断。结论超声对间质部妊娠的诊断及鉴别诊断具有重要的临床应用价值。

It has three types of sonographie representation： gestational sac type, mass type and ruptured type. There are 9 cases of gestational sac type, with 1 case in them wrongly diagnosed as angular pregnancy （8/9） ; there are 16 cases of mass type, with 1 case wrongly diag- nosed as angular pregnancy and 2 cases wrongly diagnosed as gestational trophoblastic disease （3/16）; there are 3 cases of ruptured type, all diagnosed as tubal rupture and bleeding due to ectopic pregnancy, without clear localization diagnosis. Conclusion Ultrasound is of great clinical value for interstitial tubal pregnancy diagnosis and differential diagnosis.
