高剂量盐酸氨溴索在创伤性湿肺治疗中的应用 被引量:2

摘要 目的观察高剂量盐酸氨溴索治疗创伤性湿肺的疗效。方法选取本院168例创伤性湿肺患者,按照随机数表法分为对照组79例(给予常规治疗)和治疗组89例(在常规治疗的基础上给予高剂量盐酸氨溴素100 mg静脉推注,2次/d,持续10 d)。监测两组患者肺部影像学、血气分析等生化指标以及机械通气情况,评估两组患者肺功能恢复程度以及疾病转归。结果肺部影像学结果显示治疗组有效率为85.3%,高于对照组的70.5%,差异有统计学意义(P<0.05);治疗组治疗10 d后自主呼吸频率、血氧分压(Pa O2)值以及血乳酸水平等血气指标均优于对照组,差异均有统计学意义(P<0.05);治疗组平均机械通气时间短于对照组,差异有统计学意义(P<0.05)。结论高剂量盐酸氨溴索静脉注射可以改善创伤性湿肺引发的肺功能障碍,可以作为创伤性湿肺常规治疗的辅助措施。 Objective To observe the protective role of high-dose ambroxol hydrochlorideon in the treatment of traumatic wet lung. Methods One hundred and sixty-eight patients with traumatic wet lung were randomly divided into the treatment group(n=89) and the control group(n=79). The control group received normal treatment,while the treatment group received additional high-dose ambroxol hydrochloride injection 100 mg twice per day for10 days. X-ray, blood gas analysis and mechanical ventilation were detected during the procedure, which showed the functional recovery of lung and the prognosis of disease. Results The effective rate of the treatment group was85.3%, significantly higher than 70.5% of the control group(P〈0.05). Ten days after treatment, the respiration frequency, Pa O2 and the concentration of blood lactate in the treatment group were all significantly better than those in the control group(P〈0.05). The mean time of mechanical ventilation of treatment group was shorter than the control group(t=5.965, P〈0.05). Conclusion High-dose ambroxol hydrochloride injection has protective effects on curing traumatic wet lung, which can be applied as an assisted treatment for normal treatmen.
