Characteristic imaging features of primary bone lymphoma

目的探讨原发性骨淋巴瘤（primary bone lymphoma，PLB）的影像特点，以提高对该病的诊断、鉴别诊断能力。方法回顾性分析26例经病理证实的PLB患者的X线、CT、MⅪ表现。结果23例PLB单发，分别位于股骨6例、髂骨9例、眩骨2例、胫骨2例、颞骨1例，耻骨支1例，椎体2例；3例PLB多发，1例同时累及双侧髂骨、一侧骶骨；l例股骨、胸椎及附件、肋骨同时累及；另外1例同时累及双侧股骨远端。26例患者中溶骨型17例，混合型6例，硬化型3例，病变M对表现为T1WI呈等或稍低信号，T2WI脂肪抑脂序列呈等高信号，肿瘤可呈明显均匀或不均匀强化。结论PLB多发生于中老年人，可单骨、多骨发生且具备一定的影像学特点，综合影像方法在PLB诊断和预后评估上有着重要价值。

Objective To analysis the characteristic imaging features of primary bone lymphoma and to improve the diagnostic ability of differential diagnosis. Methods 26 cases of primary bone lymphoma proved by pathology were collected and the imaging findings were retrospectively analyzed. Results Within 26 cases of primary bone lymphoma, 23 cases were solitary lesions, 3 cases were multifocal lesions, oteolytic lesions in 17 cases, sclerotic lesions in 3 cases and mixed findings in 6 cases. Most of lesions showed isointensity or low T1 signal intensity and isointensity or high signal intensity in T2WI- FS. Lesions showed significantly homogeneous or inhomogeneous enhancement. Conclusion Primary bone lymphoma as usually seen in aged patient group, can manifest as solitary or multi-focal lesions with some characteristic imaging features. The comprehensive methods in the diagnosis of primary bone lymphoma and prognosis evaluation were valuable.
