摘要
目的探讨原发性胃腺癌(gastric adenocarcinoma,GAC)组织中ERK1和E-cadherin蛋白的表达情况及其临床病理意义。方法应用免疫组织化学ElivisionTM plus法检测261例GAC组织和100例正常胃黏膜组织中ERK1和E-cadherin蛋白的表达。结果在正常胃黏膜组织中,ERK1和E-cadherin蛋白的阳性表达率分别为26.0%和100%;在GAC组中,ERK1和E-cadherin蛋白的阳性表达率分别为49.8%和41.4%,两组之间,差异有统计学意义(P<0.05)。ERK1和Ecadherin蛋白的阳性表达水平在肿瘤组织的不同分化程度、淋巴结转移与否、浸润深度及临床分期等之间差异有统计学意义(全部P<0.05);ERK1蛋白的阳性表达率与E-cadherin蛋白的阳性表达率呈负相关关系(P<0.05)。Kaplan-Meier生存分析表明,ERK1蛋白表达阳性的患者其生存时间显著低于其阴性表达患者(P<0.001);E-cadherin蛋白表达阳性的患者其生存时间显著高于其阴性表达患者(P<0.001)。多因素Cox回归分析:ERK1、E-cadherin蛋白的表达水平以及TNM分期是影响GAC患者术后生存率的独立预后因素。结论 ERK1和E-cadherin蛋白的表达水平与GAC患者肿瘤组织的分化程度、浸润深度、淋巴结转移、临床分期及预后等均有关;在GAC组织中联合检测ERK1和E-cadherin对判断GAC患者淋巴结转移及预后均有重要意义。
Objective To explore the expression of ERK1 and E-cadherin protein in the primary gastric adenocarcinoma(GAC)and their clinicopathological significance.Methods Expression of ERK1 and Ecadherin proteins was examined by immunohistochemistry in 261 specimens of GAC tissues and 100 specimens of normal gastric tissues.Results The positive expression of ERK1 and E-cadherin proteins in normal gastric tissues and GAC tissues was 26.0%,49.8%and 100%,41.4%,respectively.There were a significant differences between the two group(P<0.05).The expression of ERK1 and E-cadherin proteins was significantly associated with grades of tumors,lymph node metastasis,invasive depth and tumor-node-metastasis(TNM)stages(all P<0.05);and there was a negative relationship between the expression of ERK1 protein and E-cadherin protein(P<0.05).Kaplan-Meier analysis showed that:the survival time of ERK1-positive or ERK1-negative group was significantly different(P<0.001);the survival time of the Ecadherin positive group was significantly longer than that of the negative group.Cox regression analysis showed that expression of ERK1 and E-cadherin proteins and TNM stages were independent factors of postoperative GAC patients(P<0.05).Conclusion The expression of ERK1 and E-cadherin is related to grades of tumors,invasive depth,lymph node metastasis,TNM stages,and prognosis.The combined detection of ERK1 and E-cadherin has an important role to predict the metastasis and prognosis in GAC patients.
