Preliminary clinical application of hybrid PET/MR

摘要 目的回顾性探讨PET/MR在全身性肿瘤筛查、局部肿瘤显像及神经系统功能成像中的临床价值。方法共人组志愿者425例[男149例,女276例,年龄21∽82(平均56)岁],其中全身性肿瘤筛查58例,局部肿瘤显像264例,神经系统功能成像103例,均行全身或局部18F—FDGPET/MR检查。结果58例PET/MR全身性肿瘤筛查结果阳性49例(84.48%),均为良性病变;局部肿瘤显像中,病灶阳性显示率(含34例18F。FDGPET阴性肿瘤患者)为100%(264/264),诊断准确性为95.45%(252/264),其中56例宫颈癌PET/MR的国际妇产科联盟(FIGO)分期诊断准确性为92.86%(52/56),46例鼻咽癌患者近期疗效评价CR率为69.57%(32/46);103例神经系统功能成像中,92例(89.32%)PET/MR阳性诊断经手术及临床随访证实为真阳性。结论PET/MR一站式检查可显著提高疾病的显示及诊断的准确性,可为临床提供有价值的病变定位、定性、定量及功能信息。 Objective To retrospectively evaluate the clinical application of hybrid PET/MR in whole-body tumor screening, regional tumor imaging and central nervous system (CNS) functional imaging. Methods A total of 425 volunteers (149 males, 276 females, age: 21- 82 (mean 56) years) were en- rolled in this study, including 58 healthy subjects for whole-body PET/MR, 264 cases with tumors for re- gional PET/MR and 103 cases with CNS problems for CNS PET/MR. Results A total of 49 cases (84. 48%) undergoing whole-body PET/MR had positive results which were all proved to represent benign lesions. Regional PET/MR was positive in 100% of patients (264/264, 34 cases were XSF-FDG PET nega- tive), and the diagnostic accuracy was 95.45% (252/264). The accuracy for PET/MR staging in cervical carcinomas was 92.86% (52/56) and CR ratio of nasopharyngeal carcinomas was 69.57% (32/46). CNS PET/MR was positive in 92 cases (89.32%) which were confirmed true positive by post-operative pathology and clinical follow-up. Conclusions PET/MR might improve lesion detection rate and diagnostic accuracy. It could provide accurate lesion localization, qualification, quantification and function assessment.
