壮药“骨痹方”烫熨配合运动疗法治疗膝关节骨关节炎的效果观察

Effect of strong medicines "Gu Bi Fang" combine quadriceps exercise on knee osteoarthritis

目的观察壮药"骨痹方"烫熨配合运动疗法治疗膝关节骨关节炎(KOA)的疗效和安全性。方法选择KOA患者60例,随机分为壮药烫熨组(A组)、运动疗法组(B组)、壮药烫熨联合运动疗法组(C组)和双氯芬酸钠联合运动疗法组(D组),每组15例。烫熨疗法每天2次,运动疗法每天3次,双氯芬酸钠75 mg/次、1次/d。治疗后2、8周进行VAS评分、Lequesne指数、中医临床疗效等比较。结果治疗后各组VAS评分均较治疗前下降,以B、D组明显,B、D组与A、C组比较差异有统计学意义(P均<0.05);治疗后2、8周,Lequesne指数评分B组低于A、C、D组(P均<0.05);中医体征:在止痛效果上,A、B组较C、D组差(P均<0.05);烫疗疗法效果体现在消肿上,运动疗法效果体现在改善关节活动度上。结论壮药"骨痹方"烫熨配合运动疗法治疗KOA效果确切,安全性较高,值得临床推广应用。

Objective To investigate the therapeutic effect and safety of strong medicines hyperthermia treatment and combine quadriceps exercise for knee osteoarthritis .Methods There were 60 cases enrolled , they were divided into A , B, C, D group.Patients in group A were given strong medicines hyperthermia treatment; patients in group B were given hyperthermia treatment and combine the correct quadriceps exercise;patients in group C were given quadriceps exercise , patients in group D were given NSAIDs drug and combine the correct quadriceps exercise .Hyperthermia treatment was twice a day.Correct quadriceps exercise was 3 times a day.VAS, Lequesne index, clinical efficacy of Chinese medicine were compared after two or eight week.Results VAS value decreased after treatment in each group , especially group B, D. There were significantly different among grup B , D and group A, C （all P〈0.05）.Lequesne index in group B was higher than that in goup A, C, D.killing pain effect in group A, B were less than that in goup C, D（all P〈0.05）.hyperther-mia treatment had the vantage of anti-swelling; correct quadriceps exercise can effectively improve the range of motion of joints .Conclusion Strong medicines hyperthermia treatment combine quadriceps exercise can alleviate the symptom effec -tively, improve the joint＇s function of knee osteoarthritis patients .
