磁共振弥散张量成像在腰椎间盘突出症神经根受压中的临床应用 被引量:14

摘要 目的探讨磁共振弥散张量成像(DTI)对腰椎间盘突出症患者中相应神经根改变的临床应用价值。方法对29例经临床及影像学检查诊断的腰椎间盘突出症患者,均行常规MRI以及DTI扫描检查,通过设定感兴趣区域(ROI)的种子点对相应腰骶神经根进行纤维束示踪,并测量腰骶神经根的各向异性分数(FA)值及表观扩散系数(ADC)值。结果 DTI能完整地显示腰骶神经根的走行特点。椎间盘突出症患者病变侧神经根的ADC值[(1.772±0.499)×10-3mm2·s-1]高于健侧神经根的ADC值[(1.467±0.449)×10-3mm2·s-1]、病变侧神经根的FA值(0.249±0.031)明显低于健侧神经根FA值(0.330±0.025)(P<0.05或P<0.01)。结论腰骶神经根DTI可为临床诊治腰椎间盘突出症患者的神经根变化提供定量化数值及直观、准确的影像学依据。通过FA、ADC值可间接地反映神经根内水分子的运动,为揭示腰椎间盘突出所致的腰腿痛病因及临床相关症状的诊治提供客观的参考依据。
