氨苯砜超敏反应综合征并发Ⅱ型麻风反应

Dapsone hypersensitivity syndrome with type Ⅱ leprosy reaction

报告1例氨苯砜超敏反应综合征并发Ⅱ型麻风反应。患者因麻风病行氨苯砜、利福平、氯法齐明治疗21 d后,先后出现发热、结节性红斑、麻疹样皮损、淋巴结增大、肝酶谱升高、白细胞及嗜酸性粒细胞增高、异形淋巴细胞等症状,诊断为氨苯砜超敏反应综合征并发Ⅱ型麻风反应,二者同时发生导致病情复杂,临床上容易误诊。

A case of dapsone hypersensitivity syndrome with type Ⅱ leprosy reactiori is reported. A leprosy patient presented with high fever, erythema nodosum, morbilliform skin rash, lymphadenopathy, elevation of hepatic enzymes, leukocytosis, eosinophilia and abnormal lymphocytes after 21 days of treatment of dapsone, ifampicin and clofazimine. The diagnosis of dapsone hypersensitivity syndrome with type Ⅱ leprosy reaction was made. These two conditions occurred at the same time lead to the complicated condition, and it would be easily misdiagnosed clinically.
