过渡期护理模式在出院后中度慢性阻塞性肺疾病病人肺功能改善中的应用

Effects of transitional care model on improving pulmonary function of moderate chronic obstructive pulmomary disease patients after discharge

目的探讨过渡期护理模式对中度慢性阻塞性肺疾病病人肺功能的影响。方法将203例中度慢性阻塞性肺疾病病人按抽签法随机分为对照组(101例)和干预组(102例)。对照组采用慢性阻塞性肺疾病常规治疗、护理及健康教育,干预组在此基础上采用过渡期护理模式,对病人进行呼吸锻炼、运动管理、饮食管理及健康生活方式指导、环境控制、情感管理、用药管理、症状处理及社会支持等康复护理。比较两组病人在出院时、出院后1个月、3个月、6个月肺功能监测指标、6 min步行试验和再入院次数。结果过渡期护理模式干预后1个月、3个月、6个月,干预组病人肺功能监测指标、6 min步行试验较对照组高(P<0.01),再次入院次数较对照组低(P<0.01或P<0.05)。结论过渡期护理模式能有效改善中度慢性阻塞性肺疾病病人肺功能,延缓病情进展,降低病人再次入院次数。

Objective To evaluate the effects of transitional care model on improving pulmonary function of moderate chronic obstructive pulmomary dis- ease （COPD） patients after discharge. Methods Totally,203 patients with moderate COPD were randomly divided into the expm＇imental group （n = 102） and the control group （n = 101 ）. All patients received traditional COPD treatment, nursing and health education, and the patients in the experimental group received transitional care, including respiration training, exercise management, diet management, healthy lifestyle guidance, environment control, emotion management, med- icine management and social support. The indicators of puhnonary function,6 - minute walking test and rehospitalization number were compared at discharge, 1 month,3 months,and 6 months after discharge between the two groups. Results After intervention,indicators of pulmonary function and 6 - minute walking test in the experimental group were better than those of the control group （P 〈 0.01 ） , and the rehospitalization number was lower than that of the control group （P 〈 0.05 or P 〈 0.01 ）. Conclusion The transitional care model can improve the puhnonary function of moderate COPD patients after discharge, delay the disease progression and decrease the rehospitalization number.
