摘要 目的 探讨CT血管成像(CTA)在感染性腹主动脉瘤中的临床诊断价值.方法 对2010年10月~2013年12月期间我院经外科手术确诊的6例感染性腹主动脉瘤患者的临床及CT资料进行回顾性分析,观察感染性腹主动脉瘤的部位、大小、范围以及是否累及腹主动脉主要分支等.结果 6例腹主动脉瘤中,3例患者累及左肾动脉下至髂血管分叉水平,出现管壁多发钙化,管腔呈瘤样扩张,腔内见不规则充盈缺损;其余3例患者累及髂总动脉及髂股动脉,出现腹主动脉及髂总动脉管壁多发混合斑块,管腔呈瘤样扩张,腔内见环形充盈缺损,相应管腔不同程度狭窄.结论 CTA能够清晰显示腹主动脉瘤的病理解剖改变,是感染性腹主动脉瘤临床评价的首选影像检查方法. Objective To evaluate the clinical diagnostic value of CTA in infected abdominal aortic aneurysms. Methods The clinical and CT data of six patients surgically diagnosed infected abdominal aortic aneurysm were collected in our hospital from October 2010 to December 2013. The location, size and scope of the fumor and whether it involved the abdomi- nal aorta and other major branches were analyzed. Results Among the six patients with infected abdominal aortic aneu- rysm, there patients involoved in the left renal artery bifurcayion down to the level of iliac vessels. There were multiple calcification in the vascular wall, the lumen was aneurysm and irregular filling defect in the ensovascular. The other three patients involved in the iliac artery and femoral arteries. There were multiple calcification in teh abdominal aorta and iliac artery vascular wall. The lumen showed a tumor-like expansion and annular filling defect can be seen in the cavity. There was various degrees of stenosis in the lumina. Conclusion CTA can clearly find the pathologic anatomy changes of abdominal aortic aneurysm. Therefore, CTA is the preferred medical imaging method for the detection of abdominal aortic aneurysm.
