颈性眩晕的发病机制及诊治新进展

颈性眩晕的发病机制尚不清楚,常见的假说有颈本体觉相关性眩晕和神经血管性眩晕,国外学者多针对颈本体觉相关性眩晕行手法疗法、物理治疗或前庭康复治疗。国内学者则多针对神经血管机制行微创或手术治疗。由于本病多以主诉为主,阳性体征较少,且无特异性的辅助检查和临床试验方法,确立诊断前应排除其他可能原因引起的眩晕。目前多数有影响力的文献提供的仅是对颈性眩晕的规律性总结及临床认识,缺少对其基础领域进行深入研究的报告。

The pathogenesis of cervical vertigo is not clear. Neck proprioceptive associated vertigo and neurovascular- related vertigo are common hypotheses. Foreign scholars apply manual therapy,physical therapy or vestibular rehabilitation therapy to treat cervical vertigo. Domestic scholars often make use of surgery or minimally invasive therapy. Diagnosis of cervical vertigo depends mostly on patients＇ subjective feelings,lacking positive signs,specific laboratory examinations and clinical trials,and often relies on limited clinical experiences of clinicians. Most influential literatures just provide regularity summary and clinical knowledge of cervical vertigo,lack further research report on the basis of this field.
