NT-proBNP及CA125评价先天性心脏病患儿心功能的价值 被引量:4

Significance of NT-proBNP and CA125 in evaluating cardiac function of children with congenital heart disease

摘要 目的探讨血浆氨基末端脑钠肽原(NT-proBNP)及血清糖类抗原125(CA125)对先天性心脏病(CHD)患儿心功能评价及心力衰竭(心衰)早期诊断的临床应用价值。方法 74例CHD患儿分为CHD心衰组(A组,34例)和CHD无心衰组(B组,40例),22例门诊健康体检儿童作为对照组(C组),应用ELISA法测定血浆NT-proBNP和血清CA125浓度。结果A组血浆NT-pro-BNP浓度高于B、C组(P<0.01);A组患儿血浆NT-proBNP浓度随心衰程度的加重而升高(P<0.01)。A组患儿血清CA125浓度高于C组(P<0.01)。CHD患儿血浆NT-proBNP和血清CA125浓度分别均与改良ROSS评分呈正相关(r=0.941和r=0.690,P<0.01)。结论 CHD患儿合并心衰时血浆NT-proBNP及血清CA125浓度升高,可用于CHD患儿的心功能评价及心衰的早期诊断。 Objective To investigate the significance of N-terminal pro-BNP (NT-proBNP) and carbohydrate antigen125 (CA125) in the diagnosis of heart failure and evaluation of cardiac function of children with congenital heart disease(CHD). Methods Seventy-four children with CHD were divided into two groups of A (CHD with heart failure, 34 cases) and B (CHD without heart failure, 40 cases). Twenty-two healthy children were taken as the controls(group C). Plasma NT-proBNP and serum CA125 were detected by ELISA. Results Plasma level of NT-proBNP in group A was higher than that in groups of B and C(P〈0. 01). Plasma levels of NT-proBNP were increased as heart failure aggravated in group A(P〈0. 01). Serum level of CA125 in group A was higher than that in group C (P〈0. 01). Plasma levels of NT-proBNP and serum CA125 were positively correlated with ROSS scores(r=0. 941 and r=0. 690, P〈0. 01). Conclusion Plasma levels of NT-proBNP and serum CA125 are increased in CHD patients with heart failure,which could be taken as the indicators in the evaluation of cardiac function and early diagnosis of heart failure in children with CHD.
