常规超声与超声弹性成像评价犬前列腺内射频消融灶

摘要 目的探讨常规超声与超声弹性成像评价正常实验犬前列腺内射频消融灶的可行性。方法依次对5只雄性实验犬前列腺两侧叶进行射频消融,每侧叶消融结束后采用常规超声、超声弹性成像技术观察消融灶,获取消融灶在超声弹性图像的最大左右径、前后径和上下径,并计算消融灶体积。处死实验犬,取前列腺大体标本观察消融灶,测量并计算消融灶体积,并与弹性成像测量结果比较。结果常规超声不能准确反映犬前列腺内射频消融灶;弹性图像显示射频消融灶出现蓝色为主蓝绿相间区域。弹性成像与大体病理测量消融灶体积比较差异无统计学意义(t=1.03,P=0.33)。结论超声弹性成像能反映前列腺内射频消融灶,其图像特点表现为消融后消融区域硬度增加。常规超声在消融早期无法准确判断消融范围。 Objective To evaluate the feasibility of conventional ultrasound and elastography in evaluating lesions produced by radiofrequency ablation(RFA)in canine.Methods Both prostate side lobes of five male canine were performed RFA procedures.Conventional ultrasound and ultrasound elastography were performed immediately after RFA procedures.Subsequently,all animals were sacrificed and the prostates were gotten for pathologic analysis.The volumes of the RFA lesions obtain from elastography were calculated and compared with the pathologic results.Results The RFA lesions could not be observed accurately with conventional ultrasound.Ultrasound elastography was able to monitor the lesions in real time.After RFA,it indicated that most of the lesions had no strain.The RFA lesions were represented almost as blue or sometimes mixed with light green.There were no statistically significant differences between average lesion volume on elastography and histology(t=1.03,P=0.33).Conclusion Elastography can reflect RFA lesions of prostate,the image characteristics are as hardness increases after ablation.Conventional ultrasound can not evaluate the RFA lesion correctly in the earlier period.
