体重因素与妊娠期高血压疾病的相关性

Correlation between weight and hypertensive disorder complicating pregnancy

目的：探讨孕前BMI、孕20周前体重增长与妊娠期高血压疾病（HDP）的相关性,为HDP的治疗提供理论依据。方法：选取2012年1月-12月在北京市海淀区妇幼保健院住院分娩的产妇12 259名,收集其病历资料,采用多因素Logistic回归对相关因素进行分析。结果：孕前超重/肥胖者发生HDP的风险分别为孕前BMI正常者的2.6（2.1-3.1）和4.4（3.2-5.9）倍;孕20周前体重增长较多者发生HDP的风险是体重增长较少者的1.2（1.0-1.4）倍;孕前超重/肥胖者中孕20周前体重增长较少者HDP的发生风险低于体重增长较多者。结论：孕前超重/肥胖、孕20周前体重增长过多均是HDP的危险因素,与孕20周前体重增长较多相比,孕前超重/肥胖在HDP的发生中作用更强;减少孕20周前体重增长可在一定程度上降低孕前超重/肥胖者HDP的发生风险。

Objective： To explore the correlation between progestational body mass index （ BMI）, body weight gain before 20 gestational weeks and hypertensive disorder complicating pregnaficy （HDCP）, provide a theoretical basis for treatment of HDCP. Methods： A total of 12 259 pregnant women who gave birth to their babies in Maternal and Child Health Carelpital of Haidian District from January to December in 2012 were selected, then the data was collected, multivariate Logistic regression was used to analyze the related factors. Resuits： The risks of HDCP in overweight and obese women before pregnancy were 2. 6 （2. 1 - 3.1 ） and 4.4 （ 3.2 - 5.9） times more than those in women with normal BMI before pregnancy; the risk of HDCP in women with excessive body weight gain before 20 gestational weeks was 1.2 （ 1.0 - 1.4） times more than that in women with less body weight gain before 20 gestational weeks ; among the overweight and obese women before pregnancy, the risk of HDCP in women with less body weight gain before 20 gestational weeks was lower than that in women with excessive body weight gain before 20 gestational weeks. Conclusion. Progestational overweight/obesity and excessive body weight gain before 20 gestational weeks are risk factors of HDCP. Compared with excessive body weight gain before 20 gestational weeks, the effect of progestational overweight/obesity is more significant in occurrence of HDCP; reducing body weight gain before 20 gestational weeks can re duce the risk ：dr HDCP in progestational overweight/obese women.
