穴位注射配合吻合器痔上黏膜环切术治疗直肠前突

Effect on acupoint injection combined with PPH in treating rectocele

目的观察穴位注射配合吻合器痔上黏膜环切术(PPH)治疗直肠前突(RC)的临床疗效。方法选取2012年12月—2013年10月间我院收治的RC患者120例,随机分为观察组与对照组,各60例,2组患者均采用PPH治疗,观察组术后给予维生素B1穴位注射治疗,注射穴位包括上巨墟、足三里、神门、天枢、咳肛及背俞穴,对照组患者不进行穴位注射治疗;观察2组治疗前后症状积分、直肠肛管测压改善及临床疗效。结果观察组治疗后各时间点症状积分均显著低于对照组(P<0.05);观察组治疗9 d后肛管最大收缩压、肛管舒张压、肛管静息压改善均显著优于对照组(P<0.05);观察组治疗总好转率(96.7%)显著高于对照组(85.0%)(P<0.05)。结论穴位注射配合PPH术可显著改善RC患者临床症状,有效地保护肛门功能,提高肛管最大收缩压和肛管舒张压,降低肛管静息压。

Objective To observe the clinical efficacy of acupoint injection in combined with PPH in treating rectocele（ RC）. Methods A total of 120 patients with RC admitted in our hospital from December,2012 to October,2013 were enrolled in our study and randomized into the observation group and the control group with 60 cases in each group. All the patients in the two groups were given PPH treatment. The patients in the observation group were given Vitamin B1 acupoint injection after operation. The injection acuponints included Shangjuxu,tsusanli,Shenmen,Tianshu,cough anus,and back-shu. However,the patients in the control group were not performed with acupoint injection.The symptom scores,the improvement of anorectal manometry,and the clinical efficacy before and after treatment in the two groups were observed. Results The timing-point symptom score in the observation group after treatment was significantly lower than that in the control group（ P〈 0. 05）. The improvements of anal canal maximum systolic pressure,diastolic pressure,and resting pressure in the observation group after 9-day treatment were significantly superior to those in the control group（ P〈 0. 05）. The total improvement rate（ 96. 7%） in the observation group was significantly higher than that in the control group（ 85. 0%）（ P〈 0. 05）. Conclusion It can be concluded that acupoint injection in combined with PPH can significantly improve the RC patients＇ clinical symptoms,effectively protect the anal function,elevate the anal canal maximum systolic pressure and diastolic pressure,and reduce the anal canal resting pressure.
