脑与脊髓同时受累的瘤样炎性脱髓鞘病一例

One case of tumor-like inflammatory demyelinating disease with brain and spinal cord

目的了解瘤样炎性脱髓鞘病(TIDD)的临床表现、诊断及治疗方法。方法介绍我院收治的1例脑与脊髓同时受累的TIDD患者的临床表现、影像学特点及治疗方法。结果本病临床症状多种多样,出现头晕、头痛及肢体无力等症状,进展迅速,头颅与脊髓影像学检查可见异常信号。给予大剂量注射用甲泼尼龙琥珀酸钠冲击治疗,效果显著,预后较好。结论本病为中枢神经系统脱髓鞘疾病,临床表现多种多样,易与颅内肿瘤相混淆,激素冲击治疗有效。

Objective To investigate the clinical characteristics, diagnosis and treatment of the tumor-like inflammatory demyelinating disease with brain and spinal cord. Methods We introduced the clinical characteristics, imaging and treatment methods of the tumor-like inflammatory demyelinating disease with brain and spinal cord. Results The various symptoms of disease such as dizziness, headache and weakness, progress rapidly, it showed abnormal signals by MR imaging of brain and spinal cord. The treatment of methylprednisolone sodium sueeinate was significantly improved and had a favourable prognosis. Conclusion The disease was one of the central nervous system demyelinating disease, clinical manifestations varied, easily misdiagnosed as intracranial tumors, and corticosteroid was the effective treatment.
