横长比在评估椎基底动脉梭形动脉瘤破裂风险中作用

摘要 目的探讨动脉瘤几何形态在评估椎基底动脉梭形动脉瘤破裂风险中的作用。方法回顾性分析162例椎基底动脉梭形动脉瘤,测量其横径及长径,对数据采用t检验,分析横径、长径及两者的比值(横长比)与动脉瘤破裂的相关性。结果破裂组长径为8.36 mm±3.48 mm,小于未破裂组11.67 mm±7.37 mm(P=0.001);破裂组横长比为1.05±0.51,显著大于未破裂组0.79±0.39(P=0.003)。横长比在预测椎基底动脉梭形动脉瘤破裂风险中的最佳界点值为0.7925。结论横长比的大小与梭形动脉瘤的破裂风险存在相关性,当横长比大于0.7925时,椎基底动脉梭形动脉瘤破裂风险明显增高。 Objective The role of geometric characteristics of an aneurysm in predicting the rupture risk of fusiform vertebro-basilar artery aneurysm is investigated. Methods A Total of 162 fusiform vertebro-basilar artery aneurysms were analyzed retrospectively,and the width and length of aneurysms were measured in order to evaluate the predictive abilities of length,width and width-length ratio( WLR) for aneurysm rupture. Results The length of aneurysm in rupture group was 8. 36 mm ± 3. 48 mm,shorter than that of un-rupture aneurysm( 11. 67 mm ± 7. 37mm). The WLR in rupture group was 1. 05 ±0. 51 compared to 0. 79 ±0. 39 in un-rupture group. The cut-off value of WLR was 0. 7925. Conclusion The WLR is confirmed to correlate with the rupture risk of fusiform aneurysm. The rupture risk of fusiform vertebro-basilar artery aneurysm is increased obviously with the value of WLR over 0. 7925.
