老年慢性阻塞性肺疾病患者呼吸衰竭合并肺部感染及死亡相关因素分析

Analysis of Respiratory Failure Complicated by Pulmonary Infecetions and Death-related Factors in the Elderly COPD Patients

目的探讨老年 COPD患者呼吸衰竭合并肺部感染的原因、治疗及影响预后的相关因素。方法对 12 6例住院的老年 COPD患者呼吸衰竭合并肺部感染及死亡的临床资料进行回顾性分析 ,并和 110例非老年 COPD患者组对照。结果 12 6例呼吸衰竭合并肺部感染及死亡的相关因素有 :年龄、基础疾病与伴发症、肺部感染及病原菌、治疗和抢救不当等 ,比较肯定的认为肺部感染是最主要的危险因素。结论本组患者死亡相关因素为基础疾病、呼吸衰竭和肺部感染三者互为因果 ,但肺部感染是加重呼吸衰竭死亡的主要危险因素 。

Objective To explore the cause,treatment and the death-related factors of respiratory failure complicated by pulmonary infections in the elderly patients with COPD.Methods The clinical data of 126 elderly COPD inpatients with respiratory failure complicated by lung infections were retrospectively analyzed,and compared with the data of 110 old non-COPD patients.Results The related causes for these two complications in COPD were:age,primary disease and complications,infection of lungs and pathogens,improper treatment and rescue.The death-related causes for these patients were:pathogens,respiratory failure and infection of the lungs.Infection of the lungs was considered to be the most important and most dangerous cause.Conclusion Infection of lungs is the most dangerous death cause for the respiratory failure,rational use of antibiotic is the key point to improve the respiratory failure and reduce the death rate.
