摘要
目的研究分析无创正压通气联合纤维支气管镜吸痰治疗肺心病并肺性脑病的临床效果。方法选取2012年1月~2014年1月收治的肺心病并肺性脑病患者60例进行回顾性分析,将其分为观察组和对照组,其中对照组30例,采取常规治疗方法,观察组30例,在对照组基础上采取无创正压通气联合纤维支气管镜吸痰治疗,对治疗前、中、后行实验室指标、生命体征、脉搏、血氧饱和度、血气分析等观察监测,并作分析比较。结果观察组患者治疗前、后Pa CO2分别为(11.04±2.15)k Pa和(7.84±1.69)k Pa,Pa O2分别为(7.80±1.88)k Pa和(11.13±2.08)k Pa,通气后患者病情有显著改善(P<0.05);与对照组比较,观察组的痰菌阳性例数、死亡例数、住院天数均有明显的优势,差异均有统计学意义。结论无创正压通气联合纤维支气管镜吸痰治疗肺心病并肺性脑病的临床效果相对比常规治疗的效果较好,值得临床推广使用。
Objective Research and analysis of the clinical effect of noninvasive positive pressure ventilation combined bronchoscope sputum aspiration in treating pulmonary heart disease complicated with pulmonary encephalopathy. Methods Pulmonary heart disease from 2012 January^2014 year in January were complicated with pulmonary encephalopathy in 60 cases were retrospectively analyzed,which is divided into the observation group and the control group, including 30 cases of the control group,took conventional treatment,the observation group 30 cases,on the basis of the control group taking noninvasive positive pressure ventilation combined with fiber bronchoscope treatment of sputum aspiration,before treatment,after in,laboratory index,vital signs, pulse,oxygen saturation,blood gas analysis and observation of monitoring,analysis and comparison. Results The observation group patients before and after treatment,PaCO2 were(11.04±2.15)kPa and(7.84±1.69)kPa,PaO2 were(7.80±1.88)kPa and(11.13±2.08)kPa patients,after ventilation improved significantly(P 〈 0.05);compared with the control group,the observation group of sputum positive cases the number of the number of cases,death, hospitalization days have obvious advantages,the difference had statistical significance. Conclusion The clinical efficacy of noninvasive positive pressure ventilation combined bronchoscope sputum aspiration in treating pulmonary heart disease complicated with pulmonary encephalopathy is relatively better than conventional treatment effect,is worthy of clinical use.
