罗哌卡因局麻手术治疗老年急性嵌顿痔疗效观察

Curative Effect of Surgical Treatment of Senile Acute Incarcerated Hemorrhoids under Local Anesthesia of Ropiracaine

为观察罗哌卡因局麻下手术治疗老年急性嵌顿痔临床疗效，对28例老年基础脏器疾病多而复杂的急性嵌顿痔患者在罗哌膏因局麻下行手术治疗。结果显示，罗哌卡因局部麻醉后约5min。肛门括约肌松弛，肛门坠胀疼痛消失，水肿逐渐消退，痔核向肛内回缩，然后采用1：1消痔灵注射液行内痔注射治疗。发病24h以内痔黏膜无糜烂、坏死者治疗效果更好。术后6h肛门疼痛0级6例，Ⅰ级17例，Ⅱ级5例，平均治愈时间7．6d。治愈25例，好转3例，有效率为100％。结果表明，罗哌卡因局部麻醉能够完全满足老年急性嵌顿痔手术麻醉要求，治疗早期急性嵌顿痔安全有效，不良反应少。

Author＇s hospital gave surgical treatment to 28 cases of senile acute incarcerated hemorrhoids pa tients under local anesthesia of ropiracaine who had more and complex basic organ diseases,so as to inves tigate the clinical effect of above treatment under ropiracaine local anesthesia.As results,about 5 min after local anesthesia anal sphincter became relaxant, pain and tenesmus of anus disappeared, its edema pass gradually,hemorrhoids mass receded into anus;and then received internal hemorrhoid injection of 1 ：1 Xiaozhiling solution within 24 h after onset the patients without erosion and necrosis of hemorrhoids mucosa obtained more better effect;6 h after operation in the patient＇s with anal pain 6 cases being to 0 degree, 17 to I degree,5 to II degree;the average curative time was 7.6 days;25 cases were cured,3 of improved,el fective rate was 100Y00.Results show that ropiracaine local anesthesia can completely meet the need for per forming surgery on senile acute incarcerated hemorrhoids, especially for early acute incarcerated hemor rhoids is safe and effective with less adverse reactions.
