经皮椎间孔镜下靶向穿刺椎间盘切除联合臭氧治疗腰椎间盘突出症 被引量:10

Percutaneous endoscopic lumbar discectomy combined with ozone for therapy of lumbar disc herniaion

摘要 目的探讨经皮椎间孔镜下靶向穿刺髓核摘除术(PELD)联合臭氧治疗腰椎间盘突出症的方法与效果。方法对120例南华大学附属怀化医院2009年9月至2013年3月间收治的腰椎间盘突出症患者进行随机分组,对照组60例单纯采用PELD治疗,研究组60例联合应用PELD和臭氧治疗,比较治疗后两组患者JOA及VAS评分情况。结果治疗后观察组患者JOA评分较对照组明显上升,VAS评分较对照组明显下降,两组间比较差异有统计学意义(P<0.01)。结论采用PELD联合臭氧治疗腰椎间盘突出症可进一步促进髓核的固化,减少复发,提高椎间盘内压的下降效果,促进神经根水肿粘连的改善,进而明显减轻患者因腰椎间盘突出症引起的疼痛。 Objective To investigate the methods and efficacy of percutaneous endoscopic lumbar discectomy( PELD) combined with ozone on lumbar disc herniaion. Methods One hundred and twenty patients with lumbar disc herniaion who were treated in Huaihua hospital affiliated to Nanhua university from September 2009 to March 2013 were randomly divided into control group and observation group,with 60 cases in each group. The patients in control group received PELD therapy only,while the patients in observation group received PELD combined with ozone. After treatment,JOA and VAS ratings between the two groups were compared. Results After treatment,the JOA rating of the observation group was significant higher than that of the control group; while the VAS rating of the observation group was significant lower than that of the control group. The differences between the two groups were significanct( P all 0〈. 01). Conclusion PELD combined with ozone for lumbar disc herniation can further promote the curing of nucleus pulposus,reduce recurrence and improve intradiscal internal pressure drop effect,promote the improvement of nerve root edema adhesions,and thus significantly reduce pain on patients caused by lumbar disc herniation.
