摘要
目的探讨维生素D治疗肺源性心脏病(肺心病)并低钙血症患者的临床疗效及对血清白介素2、白介素17水平的影响。方法选取2010—2013年于河北联合大学附属医院及玉田县中医院呼吸科住院的肺心病合并低钙血症患者400例,随机分为常规组190例和维生素D组210例。同时选取20例体检健康者为对照组。两组患者均给予常规治疗,维生素D组患者在常规治疗基础上给予维生素D胶囊,共治疗8个月。测定两组患者治疗前后及对照组受检者血清白介素2、白介素17水平,计量两组患者出院后8个月内呼吸道感染次数。结果与对照组比较,常规组和维生素D组患者治疗前血清白介素2水平降低、白介素17水平升高(P<0.05),治疗后常规组和维生素D组患者血清白介素2水平较治疗前升高、白介素17水平降低(P<0.05),治疗后维生素D组血清白介素2水平高于常规组,白介素17水平低于常规组(P<0.05)。维生素D组患者呼吸道感染次数较常规组少(P<0.05)。结论维生素D可以升高血清白介素2水平、降低白介素17水平,提高气道黏膜防御能力,降低气道高反应性,治疗肺心病合并低钙血症的疗效确切。
Objective To investigate clinical effect of Vitamin D on pulmonary heart disease complicated with hypocalcemia and its impact on serum interleukin 2,interleukin 17 levels. Methods From 2012 to 2013,400 pulmonary heart disease patients complicated with hypocalcemia were chosen in the Affiliated Hospital of Hebei United University and Department of Pneumology,Traditional Chinese Medicine Hospital of Yutian County,they were were randomly divided into routine group( n= 190) and Vitamin D group( n = 210). A total of 20 healthy cases were selected as control group. Routine group and Vitamin D group given conventional therapy,and Vitamin D group given extra Vitamin D capsule,both groups treated for 8 month. Serum interleukin 2,interleukin 17 levels before and after treatment,incidence of respiratory tract infection 8 month after discharge were compared. Results Compared with control group,serum interleukin 2 level was lower,serum interleukin 17 level was higher of routine group and Vitamin D group before treatment,respectively( P〈 0. 05); after treatment,serum interleukin 2level was higher,serum interleukin 17 level was lower than before treatment of routine group and Vitamin D group,respectively( P〈 0. 05),and serum interleukin 2 level was higher,serum interleukin 17 level was lower of Vitamin D group than routine group,respectively( P〈 0. 05). The incidence of respiratory tract infection of Vitamin D group was less than that of routine group( P 〈0. 05). Conclusion Vitamin D can increase the serum interleukin 2 level and,reduce serum interleukin 17 level and airway hyperresponsiveness,has certain clinical effect on pulmonary heart disease complicated with hypocalcemia.
