术前认知干预对老年患者脊柱术后谵妄的影响

Effect of preoperative cognitive intervention on the postoperative delirium in elderly patients after spinal surgery

目的:探讨术前认知干预对老年患者脊柱术后谵妄的影响。方法:将60例行脊柱手术的老年患者随机分为观察组和对照组各30例,对照组给予常规护理干预,观察组在此基础上给予术前认知干预,比较两组干预效果。结果:两组术后6 h(T1)、12 h(T2)、24 h(T3)、48 h(T4)、72 h(T5)简易智力状况检查量表(MMSE)评分与术前第1天(T0)比较差异有统计学意义(P<0.05),观察组T1、T2、T3、T4、T5 MMSE评分与对照组比较差异有统计学意义(P<0.05);两组术后谵妄发生情况比较差异有统计学意义(P<0.05)。结论:术前认知干预能明显降低老年患者脊柱术后谵妄发生率。

Objective： To explore the effect of preoperative cognitive intervention on the postoperative delirium in elderly patients after spinal surgery. Methods： 60 elderly patients who received spinal surgery were randomly divided into the observation group and the control group（ 30 cases in each group）. The patients in the control group were given routine nursing intervention and the patients in the observation group were provided extra preoperative cognitive intervention,the intervention effect was compared between the two groups. Results：The MMSE score was significantly different in 6 hrs.（ T1）,12 hrs.（ T2）,24 hrs.（ T3）,48 hrs.（ T4） and 72 hrs.（ T5） after operation compared with that on the first day before operation in the two groups（ P〈0. 05）; the difference was statistically significant between the two groups in the comparison of MMSE score of T1,T2,T3,T4 and T5（ P〈0. 05）; the incidence of POD was significantly different between the two groups（ P〈0. 05）. Conclusion： Implementation of preoperative cognitive intervention can significantly reduce the incidence of postoperative delirium in elderly patients with spinal surgery.
