2004-2013年湖南省孕产妇死亡情况分析

Analysis on the causes of maternal deaths in Hunan Province in 2004-2013

目的研究湖南省孕产妇死亡动态变化趋势及死亡原因,为降低湖南省孕产妇死亡率制定干预措施提供科学依据。方法回顾性分析2004-2013年湖南省孕产妇死亡资料。结果 2004-2013年湖南省孕产妇死亡率整体呈下降趋势,由2004年的43.74/10万下降到2013年的15.95/10万。产科出血为湖南省孕产妇首位死亡原因,以产科出血为主的直接产科因素死亡的比例由81.25%下降到39.2%,间接产科因素由18.75%上升到60.8%。影响孕产妇死亡的因素主要有孕产妇的年龄、文化程度、经济水平、城乡分布情况、孕产期保健情况、是否住院分娩和各级医疗保健知识技能。结论 2004-2013年湖南省孕产妇死亡率呈下降趋势,为进一步降低孕产妇死亡率,应加强对妊娠合并内科疾病的诊断和治疗,提高助产医务人员处理产科危急症的能力,加大宣传力度,提高住院分娩率,增强群众的保健意识。

Objective To study the dynamic change trend and causes of maternal deaths in Hunan Province,and to provide scientific evidence for formulating intervention measures and reducing maternal mortality in Hunan Province. Methods The data about maternal deaths in Hunan Province during 2004-2013 were retrospectively analyzed. Results The overall maternal mortality in Hunan Province from 2004 to 2013 showed a downward trend. The maternal mortality decreased from 43. 74 /100,000 in 2004 to 15. 95 /100,000 in 2013. The first cause of maternal deaths in Hunan Province was obstetric hemorrhage. The proportion of maternal deaths induced by the direct obstetric factors which gave priority to obstetric hemorrhage dropped from 81. 25% to 39. 2%,while those of maternal deaths induced by the indirect obstetric factors increased from 18. 75% to 60. 8%. The major factors influencing maternal deaths were the maternal age,educational background,economic level,urban and rural distribution,maternal health care,whether in the hospital childbirth and health care knowledge and skills. Conclusions The maternal mortality in Hunan Province during 2004-2013 shows a decreasing tendency.The results suggest that more attention should be paid to strengthening the diagnosis and treatment of pregnancy combined with internal medical diseases,improving the ability of basic medical staff to deal with obstetric critical diseases,enhancing the related publicity,improving the hospitalization delivery rate and intensifying the health consciousness of population so as to further reduce the maternal mortality.
