对骨性安氏Ⅲ类错采用上颌前方牵引矫治效果与骨龄关系研究

The Relationship between Effects of Skeletal Class III Malocclusion through Maxillary Protraction and Different Skeletal Maturation

目的：考察对骨性安氏Ⅲ类错采用上颌前方牵引矫治效果与骨龄关系。方法：收集2010年1月-2013年4月笔者所在医院收治的骨性安氏Ⅲ类错患者55例，根据头颅侧位片上的颈椎成熟度，将患儿分为三组，A组18例颈椎成熟度Ⅰ-Ⅱ期，B组20例颈椎成熟度Ⅲ期，C组17例颈椎成熟度Ⅳ期。结果：A、B、C三组患儿矫治时间分别为（4.0±0.5）个月、（5.3±0.4）个月、（8.9±1.0）个月。三组矫治时间比较差异有统计学意义（F=250.0，P〈0.01）。组间比较，SNA、SNB和A-OLp矫治前后差异无统计学意义（P〉0.05），U1-NA矫治后差异有统计学意义（P〈0.05），C组U1-NA增加较A组和B组明显（P〈0.01），A组变化最小，即C组上中切牙唇倾角最大。组内比较，三组SNA、ANB、U1-NA和A-OLp矫治前后差异有统计学意义（P〈0.05或P〈0.01），A组和B组SNB矫治前后比较差异有统计学意义（P〈0.05或P〈0.01），其中A组矫治前后差异显著（P〈0.01）。结论：骨性安氏Ⅲ类错采用上颌前方牵引矫治应注重骨龄评价。

Objective：To investigate the relationship between effects of skeletal class Ⅲ malocclusion through maxillary protraction and different skeletal maturation.Method：From January 2010 to April 2013,55 cases of skeletal classⅢmalocclusion were enrolled in our study and separated into 3 groups according to skeletal maturation,18 cases of skeletal maturation classⅠ-Ⅱas A group,20 cases of skeletal maturation classⅢas B group and 17 cases of skeletal maturation class Ⅳas C group.Result：There was a significant difference in the time of maxillary protraction between 3 groups,A,B,C groups were （4.0±0.5）,（5.3±0.4） and （8.9±1.0）months（F=250.0,P〈0.01）.SNA,SNB and A-OLp did not changed before and after treatment in three groups（P〉0.05）,U1-NA was different among three groups,C group was increased significantly（P〈0.01）.Compared with before treatment,SNA,ANB,U1-NA and A-OLp changed after treatment （P〈0.05 or P〈0.01）,SNB were different before and after treatment in A and B groups.A group had more obvious change between before and after treatment（P〈0.01）.Conclusion：The performing of bone age in the treatment of skeletal ClassⅢmalocclusions must be emphasized.
