摘要
目的对浙江省不同地区上送的分离株进行NDM-1基因筛检,以了解NDM-1基因携带菌存在现状。方法应用定量PCR及PCR分别对菌株进行NDM-1基因筛检、MLST分型及其他耐药基因检测;使用VITEK 2仪器测试MIC耐药表型;运用改良Hodge实验及EDTA-增强协同确认试验检测碳青霉烯酶表型。结果经对1 617株菌筛检,仅从1株ST20型鲍曼不动杆菌中检出NDM-1基因。该菌对Amikaci、Gentamicin、Tobramycin、Trimethoprim/Sulfa敏感,而对其他15种药物均显示耐药,另携带OXA-23、SPM,CARB,TEM,aadA1,AmpC,aphA1,OXA-51耐药基因。结论浙江省首次检出NDM-1鲍曼不动杆菌,提示应加强该类菌株的监测检测力度。
Objective To screen the new Delhi metallo-β-lactamase-1(NDM-1) producing strains from different regions of Zhejiang province and investigate the prevalence of blaNDM-1 positve isolates.Methods General PCR and real-time quantitative PCR were applied to screen the NDM-1 gene in all collected isolates;Minimum inhibitory concentration(MIC)test,modified Hodge test and imipenem(IPM)-EDTA double-disk synergy test were performed to confirm the phenotype of blaNDM-1positve isolates.Multilocus sequence typing(MLST) and antibiotic resistance genes were also tested for the blaNDM-1positve isolates.Results Only 1 NDM-1 producing ST20 Acinetobacter baumannii strain was identified among 1 617 strains from different regions of Zhejiang province.The blaNDM-1positive strain was susceptible to amikacin,gentamicin,tobramycin and trimethoprim/sulfamethoxazole,while resistant to other 15 antibiotics.This strain was also found carrying drug resistant gene,such as blaOXA-23,blaSPM,blaCARB,blaTEM,aadA1,blaAmpC,aphA1 and blaOXA-51.Conclusion A NDM-1 producing ST20 A.baumannii strain carrying several antimicrobial resistant genes was first found in Zhejiang province,indicating the continuous surveillance for monitoring blaNDM-1positive strains is necessary.
