H型高血压特点及与左室肥厚的关系 被引量:5

摘要 目的研究H型高血压的特点并探讨其与左室肥厚的关系。方法选择2014年1—7月门诊及住院部的30例伴高同型半胱氨酸(homocysteine,Hcy)血症的高血压患者为H型高血压组,30例血清Hcy浓度正常的高血压患者为普通高血压组,行24 h动态血压监测(ambulatory monitoring of blood pressure,ABPM)比较两组平均脉压差、高血压晨峰(morning blood pressure surge,MBPS)、血压变异性(blood pressure variability,BPV)及血压变化节律等指标,同时比较两组左室质量指数(left ventricular mass index,LVMI),组间数值变量资料的比较用独立样本t检验,组间分类变量的比较用随机设计的χ2检验或Fisher精确概率法,利用多元线性回归分析高血压病左室肥厚的相关因素,P<0.05为差异有统计学意义。结果 H型高血压组的脉压差、"非杓型"血压变化曲线的发生率[(67.1±16.9)mm Hg(1 mm Hg=0.133 k Pa)、36.7%]显著高于普通高血压组[(57.9±16.6)mm Hg、16.7%],而夜间收缩压(systolic blood pressure,SBP)下降率、舒张压(diastolic blood pressure,DBP)下降率[(11.2±9.0)%、(12.3±7.9)%]均显著低于普通高血压组[(15.9±8.7)%、(16.3±8.3)%],差异均有统计学意义(均P<0.05)。H型高血压组男、女患者的左室质量指数(left ventricular mass index,LVMI)[(119.0±19.0)、(106.4±22.2)g/m2]均较普通高血压组[(108.8±20.9)、(93.3±16.5)g/m2]高,且差异均有统计学意义(均P<0.05)。多元线性回归分析显示:男性受试者LVMI与血浆Hcy浓度、24 h SBP、夜间SBP下降率存在显著相关性(r=0.832、0.360、-79.291),差异均有统计学意义(均P<0.05);而女性受试者LVMI只与血浆Hcy浓度、夜间SBP下降率存在显著相关性(r=0.873、-77.924),差异均有统计学意义(均P<0.05)。结论 H型高血压脉压差大,且夜间血压下降率低,导致血压波动呈"非杓型"样曲线,而高血浆Hcy浓度、24 h SBP升高、夜间SBP下降率低可能导致左室肥厚。
