HPV在育龄妇女生殖道病原体感染分布及其基因分型 被引量:7

摘要 目的探讨人乳头瘤病毒(HPV)在育龄妇女生殖道病原体感染分布情况以及基因分型特点。方法收集2013年度来我院妇产科就诊及体检旳育龄妇女,阴道分泌物主要检查滴虫、真菌、淋球菌、细菌、衣原体和支原体,前三者采用涂片法,解脲支原体和人型支原体采用固体培养法,衣原体抗原检查采用乳胶免疫层析法;宫颈刮片标本采用核酸扩增和反向点杂交技术对23种HPV基因型进行检测,然后对检测结果进行分析。结果育龄妇女阴道分泌物标本病原体感染率为39.32%(7896/20080),宫颈刮片HPV感染率为35.09%(634/1807)。HPV感染中单一感染23.19%(419/1807),二重感染6.53%(118/1807),三重感染3.21%(58/1807),四重感染1.16%(21/1807),四重以上感染1.00%(18/1807),单一感染组与其他组比较差异具有统计学意义(P〈0.05)。高危中以HPV52、58、16较为常见,低危中以HPV43、6型较为常见;以25-35岁年龄段阳性率较高,其他次之。结论育龄妇女生殖道病原体感染比例较高,应引起高度重视。 Objective To explore the distribution and genotype of human Papilloma virus( HPV) in women of childbearing age. Methods The vaginal and cervical scraping samples from women of childbearing age in 2013 year in our hospital were collected and detected for trichomonad,fungi,bacteria,mycoplasma,chlamydia,and Neisseria gonorrhoeae. The cervical scraping samples were tested by using nucleic acid amplification and reverse dot blot hybridization technique to detect the 23 HPV genotypes,and the results were analyzed. Results The rate of pathogen infections was 39. 32% for the vaginal secretion samples,and that of HPV infection was 35. 09% for cervical scraping smears. The HPV infections included 23. 19% of single infection,6. 53% of double infection,3. 21% of triple infection,1. 16% of quadruple infection,and 1% of multiple infection by more than four pathogens. There was statistically significant difference between single infection group and the other groups( P 0. 05).The genotypes HPV52,58 and 16 were common in high-risk infections,while HPV43 and 6 were common in lowrisk infections. The highest positive rate was seen in ages of 25 to 35 years. Conclusion There is a high rate of pathogen infections by HPV genotypes 52,58,16,43 and 6 in women of childbearing age,which should be paid high attention.
