内镜治疗大肠息肉814例方法及疗效观察

Methods and effects of endoscopic therapy in 814 cases with colorectal polyps

目的探讨内镜治疗大肠息肉的方法及临床疗效。方法回顾性分析经内镜治疗的814例大肠息肉患者的临床资料,根据息肉的不同大小和形态,采取恰当的治疗方法。结果 814例患者的2645枚息肉均成功切除,活检钳钳除492枚,高频电切除1085枚,氩离子凝固术460枚,内镜下黏膜切除术553枚,尼龙绳套扎联合高频电切除24枚,钛夹钳夹联合高频电切除6枚,分片切除术8枚,内镜黏膜下剥离术17枚。2352枚息肉行病理检查,腺瘤性息肉1579枚,错构瘤性息肉7枚,炎症性息肉574枚,增生性息肉191枚,息肉癌变2枚,再追加外科手术治疗。术中出血14例,无1例发生穿孔。结论选择合适的治疗方法,能在有效切除息肉的同时减少并发症的发生。

Objective To investigate the methods and clinical effect of endoscopic treatment of colorectal polyps. Methods The clinical data of g14 patients with colorectal polyps who were treated by endoscopy were retrospectively analyzed. To perform the correct therapy according to the various size and morphous of polyps. Results Totally 2 645 polyps in 814 cases were all successfully removed, 492 with biopsy forceps, 1085 with high -frequency electric resection, 460 with argon plasma coagulation, 553 with endoscopic mucosal resection, 24 with nylon endoloop ligature in combination with high -frequency electric resection, 6 with titanium clips in combination with high -frequency electric resection, 8 with endoscopic piecemeal mucosal resection, 17 with endoscopic submucosal dissection. 2352 polyps received pathologi- cal examination, 1579 were adenomatous polyps, 7 were hamartomatous polyps, 574 were inflammatory polyps, 191 were hyperplastic polyps, canceration of colorectal polyps in 2 patients were detected, and they received surgical operation treatment additionally. Operative bleeding occurred in 16 cases, no perforation occurred. Conclusion The appropri- ate selection of treatment methods can remove polyps effectively, and reduce the complications at the same time.
