磁共振成像对四肢软组织脂肪肉瘤的临床诊断价值和病理对照分析

The clinical diagnosis value and pathological contrast analysis of MRI applying to extremities liposarcoma

目的探讨四肢软组织脂肪肉瘤的不同病理类型的磁共振成像(MRI)表现。方法收集经手术病理证实的四肢软组织脂肪肉瘤12例,对其MRI影像学表现与病理结果进行回顾性分析。结果 12例脂肪肉瘤中,分化良好型5例,占42%,黏液型3例,占25%,去分化型2例,占17%,多形型2例,占17%。MRI表现为T_1WI呈低、等、高混杂信号,T_2WI则以高信号为主,压脂后T_1WI、T_2WI高信号的脂肪信号呈低信号。结论 MRI可明确显示脂肪肉瘤的位置、形态、浸润范围与周围组织的解剖关系,根据脂肪肉瘤的MRI表现可以推测肿瘤的组织学分化程度,为临床提供准确、可靠的信息。

Objective To discuss MRI manifestations of different pathologic types of extremities liposarcoma.Methods The MRI manifestations of 12 cases with extremities liposarcoma proved by surgery and pathology were analyzed retrospectively.Results Among the 12 cases of extremities liposarcoma,there were 5 cases（42%） of welldifferentiated liposarcoma,3 cases（25%） of myxoid liposarco,2 cases（17%） of dedifferentiated liposarcoma,2 cases（17%） of pleomorphic types.The MRI manifestation features of extremities liposarcoma were mixed with low,medium and high intensity signal on T_1WI,and mainly high intensity signal on T_2WI.The high intensity fat signal in T_1WI and T_2WI turned to low signal after fat suppression.Conclusion The location,form,tumor aggressiveness and the relationship with anatomic relationship to surrounding tissues of the extremities liposarcoma,can be seen clearly by MRI,thus providing accurate and reliable information for clinical medicine.
