腮腺Warthin瘤的CT诊断与病理对照分析

Warthin tumor of parotid gland:CT imaging and histopathologic correlation

目的分析腮腺Warthin瘤CT特征并与病理结果对照分析,以提高本病CT诊断水平。方法回顾性分析26例37个经手术与病理证实的腮腺Warthin瘤临床资料和CT表现。26例全部行CT平扫加增强检查。结果单发20例20个病灶,多发6例17个病灶,位于腮腺浅叶后下极27个病灶。大小8mm×6mm^33mm×30mm,边界清楚33个,部分边界模糊4个病灶,23个病灶内可见多发囊变区,肿瘤周边部位裂隙状囊变13个。增强扫描肿瘤重度强化28个,中度强化9个,见"贴边血管"征或"包绕血管"征26个。镜下肿瘤由上皮成分及淋巴样组织构成,上皮组织排列成大小不等、形状不一的腺管或囊腔,腺管间见数量不等的淋巴样组织伴有淋巴滤泡形成。结论 CT表现能在一定程度上反映腮腺Warthin瘤的病理特征,对本病的诊断和鉴别诊断具有重要价值,但确诊仍需依赖病理学检查。

Objective To analyze CT features of Warthin tumor of parotid gland and to compare these with histopathology results.Methods CT features and clinic information of 26 patients（37 lesions）with Warthin tumor of parotid were analyzed,which were proved pathologically.Plain and contrast enhanced CT were performed in 26 patients.Results 20 patients had solitary lesion,6 patients had multiple lesions（17）.27 lesions were in superficial lobe area,ranged from8mm×6mm^33mm×30mm.33 lesions had clear boundaries,4 lesions had unclear boundaries.23 lesions had inhomogeneous appearance with low-density areas,13 lesions had slit shaped low-density,28 lesions had severe enhancement,9 lesions had moderate enhancement,and 26 patients had surrounding vessels.Microscopically,tumors were consisted of epithelium and lymphnoid tissue,epithelium tissue arranged crypts with different size and shape,quantity different lymphnoid tissue among crypts with lymphoid follicles.Conclusion To some extent,CT features of Warthin tumor of parotid gland were correlated with histopathology,however,histopathology examinations were needed to confirm the diagnosis.
