运动疗法配合肩关节松动术治疗肩周炎

Effect of joint mobilization combined with exercise therapy on frozen shoulder

目的 探讨运动疗法配合肩关节松动术治疗肩周炎的临床效果.方法 将80例肩周炎患者随机平均分为对照组与观察组,每组40例.对照组仅采用运动疗法,观察组在此基础上配合肩关节松动术.比较两组治疗前后VAS及C-M法评分、临床疗效、治疗前后ROM变化情况及SF-36生活质量评分.结果 ①两组患者治疗前后VAS及C-M法评分相比差异均有统计学意义（P＜0.05）,且观察组治疗后VAS评分显著小于对照组,C-M法评分显著大于对照组（P＜0.05）;②对照组临床总有效率为70.00％,低于观察组的90.00％ （P ＜0.05）;③两组患者治疗后前屈、外展及后伸ROM度数比较差异均有统计学意义（P＜0.05）,且观察组治疗后ROM度数均大于对照组治疗后（P＜0.05）;④观察组治疗后SF-36生活质量总得分为（172.10±35.93）分,显著小于对照组的（126.12±23.20）分（P＜0.05）.结论 运动疗法配合肩关节松动术治疗肩周炎疗效显著,患者生活质量得到显著提升,应在临床上加以推广及应用.

Objective To investigate the clinical effect of exercise therapy combined with joint mobilization on frozen shoulder.Methods Eighty patients with frozen shoulder were randomly divided into control group and observation group according to the balloting,with 40 cases in each group.The patients in control group were given exercise therapy,the patients in observation group were treated with joint mobilization on this basis.The VAS score and the CM method,clinical efficacy,ROM changes,and SF-36 quality of life scores of the two groups before and after treatment was compated.Results ① Before and after treatment,the difference of the VAS score and the CM method was statistically significant （P 〈 0.05）,and the VAS score of the observation group after treatment was significantly lower than the control group,CM method score was significantly greater than that in the control group （P 〈 0.05） ; ② The total effective rate of the control group was 70.00%,which was significantly lower than that of the observation group（90.00%） （P 〈0.05） ; ③After treatment,the difference of flexion,abduction and extension ROM degree were statistically significant（P 〈 0.05）,and the degree of post-treatment group were higher than that in the control group ROM after treatment（P 〈 0.05） ; ④The SF-36 quality of life for the total score [1 （172.10 ± 35.93）] points] of observation group was significantly lower than that of the control group[1（126.12±23.20） points] （P 〈0.05）.Conclusions The effect of exercise therapy combined with joint mobilization on frozen shoulder loose is significant,the quality of life of patients has been significantly improved,so it should be promoted and applied in clinical practice.
