关于开颅蛛网膜囊肿切除并脑池开放术疗效的系统评价

Outcome of open craniotomy for cyst fenestration for intracranial arachnoid ayst： a systematic review

目的 研究开颅蛛网膜囊肿切除并脑池开放术的疗效.方法 应用系统评价的方法检索Medline中1990年1月至2014年8月的文献,两位研究员独立完成文献筛选、数据提取,并且计算术后症状缓解率、影响学改善率、并发症发生率及再手术率.结果 共纳入15篇研究624例患者,Meta分析结果显示开颅蛛网膜囊肿切除并脑池开放术症状缓解率、影像学缓解率、并发症发生率、再手术率及95％置信区间分别为88％ （82％ ～92％）、77％（66％～86％）、15％（12％ ～ 19％）、17％（9％ ～ 30％）.结论 开颅蛛网膜囊肿切除并脑池开放术对于颅内蛛网膜囊肿有良好的治疗效果.

Objective To evaluate the effectiveness of open craniotomy on cyst fenestration for intracranial arachnoid cyst.Methods The Medline database from January 1990 to August 2014 was searched.Two researchers screened literatures independently,assessed the quality of the included studies and extracted the data.Then R statistical software was used to calculate rate of improvement in symptoms,rate of improvement in neuroimaging,rate of complications and rate of reoperation.Results A total of 15 studies were included,including 624 patients.Weighted summary rates and 95% CI of rate of improvement in symptoms,rate of improvement in neuroimaging,rate of complications and rate of reoperation were 88% （82%-92%）,77% （66%-86%）,15% （12%-19%）,17% （9%-30%）,respectively.Conclusions The effect of open craniotomy on cyst fenestration for intracranial arachnoid cyst is good.
