精蛋白锌重组赖脯胰岛素混合注射液不同给药方案对老年2型糖尿病患者血糖波动的影响

Analysis of the Influence of Different Dosing Regimens of Humalog 50 on Blood Glucose Fluctuation in Elderly Patients with Type 2 Diabetes

目的探讨精蛋白锌重组赖脯胰岛素混合注射液不同给药方案对老年2型糖尿病患者血糖波动的影响。方法选取2011年7月至2013年7月我院诊治的老年2型糖尿病患者50例,随机分为试验组与对照组,各25例。对照组患者给予精蛋白锌重组赖脯胰岛素混合注射液2次/d,试验组患者给予精蛋白锌重组赖脯胰岛素混合注射液3次/d;比较两组患者的血糖波动情况。结果试验组患者精蛋白锌重组赖脯胰岛素混合注射液日均总量为(41±8)U,对照组为(42±6)U,两组比较,差异无统计学意义(P>0.05)。试验组患者的血糖达标时间明显长于对照组;试验组患者平均血糖漂移幅度为(3.2±1.1)mmol/L,明显低于对照组的(5.4±1.7)mmol/L;试验组患者日间血糖平均绝对差为(1.7±0.8)mmol/L,明显低于对照组的(2.5±1.2)mmol/L;两组比较,差异均有统计学意义(P<0.05)。结论在血糖达标条件下,精蛋白锌重组赖脯胰岛素混合注射液3次/d与2次/d比较,日均胰岛素使用量无较大差异,但对血糖波动的影响相对较小,能减少低血糖的发生率。

Objective To investigate the protamine zinc recombinant insulin lispro mixed injection of differentdosage regimen in patients with blood glucose fluctuation effect on elderly patients with type 2 diabetes mellitus. Methods 50 elderly patients with type 2 diabetes from 2011 July to 2013 July in our hospital were randomly divided into experimental group and control group,each of 25 cases.Patients in control group were treated with protamine zinc recombinant lispro insulin injection 2 times a day,the test group were treated with protamine zinc recombinant lispro insulin injection 3 times a day;compared two groups of patients with blood glucose fluctuation. Results In the test group protamine zinc recombinant lispro insulin injection daily for the total（41±8）U,the control group（42±6）U,two groups,no significant difference（P〉0.05）.Blood glucose time of test group was longer than that in the control group;patients in experimental group mean amplitude of glycemic excursion for（3.2±1.1）mmol/L,significantly lower than the control group（5.4±1.7）mmol/L;in the test group blood sugar for daytime mean absolute difference（1.7±0.8）mmol/L,obviously lower than that of control group（2.5±1.2）mmol/L;the two groups,the difference was statistically significant（P〈0.05）.Conclusion In the glucose standard conditions,protamine zinc recombinant lispro insulin injection 3 times a day and 2 times a day to compare daily insulin use,without obvious difference,but the effect on blood sugar fluctuations is relatively small,can reduce the incidence of hypoglycemia.
