反复呼吸道感染、哮喘患儿血清维生素A水平的研究 被引量:39

摘要 目的探讨反复呼吸道感染哮喘患儿血清维生素A水平,了解其与免疫功能的相关性。方法选取2012年7月至2013年2月间于郴州市第一人民医院儿童医院呼吸内科就诊并治疗的反复呼吸道感染患儿72例(呼吸道感染组)及哮喘患儿25例(哮喘组),同时选取40例健康小儿为对照组,对比分析3组间VitA水平及免疫指标间的差异,同时进行相关性分析。结果与对照组相比,反复呼吸道感染患儿及哮喘患儿的VitA水平均显著下降(P<0.05);对于免疫指标的检查,与对照组相比,反复呼吸道感染患儿除IgM水平外,其余各项指标间差异均具有统计学意义(P<0.05),而对于哮喘患儿而言,仅补体C3水平间的差异具有统计学意义(P<0.05);经Pearson相关性分析发现,反复呼吸道感染患儿血清VitA与IgA、CD3+细胞数、CD3+CD4+细胞数、CD4+/CD8+比例间存在正相关性,与CD3+CD8+细胞数目间呈现负相关性;而对于哮喘患儿而言,血清VitA与各项免疫指标间均未发现明显的相关性。结论 VitA的缺乏可能导致机体免疫功能的减弱,降低儿童的免疫力,其可能是造成反复呼吸道感染及哮喘发病的重要原因,临床中应加强对于VitA的检查。 Objective To investigate the serum level of vitamin A in children with recurrent respiratory tract infections or intrinsic asthma,and to understand its correlation with immune function.Methods Seventy-two children with recurrent respiratory tract infections and 25 children with intrinsic asthma who were treated in the Department of Respiratory Medicine of Chenzhou First People's Hospital between July 2012 and February 2013 were selected in this study.In addition,40 healthy children were selected as controls.Differences in serum vitamin A level and immune indices were compared among the three groups.The correlation between vitamin A level and immune function was analyzed.Results Compared with controls,serum vitamin A level significantly decreased in children with recurrent respiratory tract infections or intrinsic asthma(P〈0.05).Furthermore,there were significant differences in immune indices,except IgM,between controls and children with recurrent respiratory tract infections(P〈0.05).However,only C3 complement was obviously different between controls and children with intrinsic asthma(P〈0.05).Pearson correlation analysis showed that serum vitamin A level was positively correlated with IgA,number of CD3^+cells,number of CD3^+CD4^+cells and ratio of CD4^+/CD8^+,but negatively correlated with the number of CD3^+CD8^+cells in children with recurrent respiratory tract infections.No obvious correlation was found between vitamin A level and immune indices in children with intrinsic asthma.Conclusion The deficiency of vitamin A may lead to an impairment of immune function in children,which may be an important cause of recurrent respiratory tract infections and intrinsic asthma.Therefore,clinical examination of serum vitamin A level should be strengthened.
