综合护理干预在重症颅脑损伤昏迷患者气管切开术中的应用价值研究

A Research on Application Value of Comprehensive Nursing Intervenes in Tracheotomy for Coma Patients Attacked by Severe Craniocerebral Injury

目的：探讨综合护理干预措施在重症颅脑损伤昏迷患者气管切开术中的应用效果.方法：选择某院2013年8月-2014年8月期间收治的重症颅脑损伤昏迷行气管切开术患者共89例作为研究对象,并按数字表法随机分为两组.其中对照组患者（n=43）仅给予常规护理,而观察组患者（n=46）则给予综合护理措施干预,观察两组患者护理干预前后的神经功能评分差异,比较两种护理方法下的护理满意度及并发症发生情况.结果：干预前两组患者的神经功能评分差异无统计学意义（P〉0.05）,而护理干预后,两组神经功能评分均显著改善,观察组的改善程度更优;另外,观察组的护理满意度高达97.83%（45/46）,并发症发生率低至6.52%（3/46）,均显著优于对照组的74.42%（32/43）、20.93%（9/43）,两组间差异比较具有统计学意义（P〈0.05）.结论：综合护理干预措施应用于重症颅脑损伤昏迷患者的气管切开术中,可有效改善患者的神经功能,提高临床满意度,降低并发症的发生,其效果显著优于常规护理,值得在临床推广使用.

Objective:To investigate the application effect of comprehensive nursing intervenes in trache‐otomy for coma patients attacked by severe craniocerebral injury .Methods:Select 89 cases of patients who were attacked by severe craniocerebral injury and comatose in some hospital and were accepted tracheotomy during the period from August 2013 to August 2014 as the study object and randomly divide them into 2 group according to number table .Treat the control group(n=43) with conventional nursing and the observa‐tion group(n=46) were intervened with comprehensive nursing .Compare the difference of neurological sys‐tem score of the patients in the two groups before and after nursing ,compare the nursing satisfaction degree and the occurance of complications when nursing with these two nursing modes .Results:Before intervening , the difference of the neurological system score of the two groups was not statistically significant ,(P>0 .05) , after nursing ,the score of the two groups was improved ,but the improvement in the observation group was more significant .Besides ,the satisfaction degree of the observation group was up to 97 .83% (45/46) ,and the occurance of complications declined to 6 .52% (3/46) ,which were all superior to that of the control group . Comparatively ,the differences were statistically significant ,(P<0 .05) .Conclusion:Applying comprehensive nursing to tracheotomy for patients who were attacked by severe craniocerebral injury and were comatose can effectively improve the neurological function of patients and clinical satisfaction degree and lower the occur‐ance of complication ,it more effectivecompared with conventional nursing ,it is worthy of clinical application and promotion .
