消疣方对复发性尖锐湿疣患者外周血单核细胞Toll样受体1、3表达的影响

Effect of Xiaoyou Recipe on Expression of Toll-like Receptor 1, 3 in Peripheral Blood Mononuclear Cells of Patients with Recurrent Condyloma Acuminatum

【目的】观察消疣方治疗复发性尖锐湿疣的临床疗效以及对外周血单核细胞中Toll样受体1、3(TLR1、TLR3)表达的影响。【方法】将60例确诊患者随机分为治疗组和对照组各30例,2组患者均先给予微波消疣,之后,治疗组给予消疣方口服和外洗患处;对照组给予胸腺肽肠溶胶囊口服,舒乐宁外洗。均治疗1个月,追踪随访6个月。于治疗前和治疗后第90天,记录皮疹复发情况,并采用双色免疫荧光抗体染色流式细胞术,观察患者外周血单核细胞的TLR1、TLR3表达水平的变化。【结果】(1)治疗组痊愈率(83.3%)优于对照组(56.7%)(P<0.05),复发率(16.7%)低于对照组(43.3%)(P<0.05)。(2)治疗前,2组TLR1、TLR3表达阳性率比较,差异均无统计学意义(P>0.05)。治疗后,2组TLR1、TLR3表达阳性率均较治疗前显著上升(P<0.05),但2组间比较,差异均无统计学意义(P>0.05)。【结论】消疣方与胸腺肽肠溶胶囊均可能通过提高TLR1、TLR3表达,刺激机体免疫应答,打破人乳头瘤病毒(HPV)的免疫逃逸机制,降低尖锐湿疣的复发率,且消疣方疗效优于胸腺肽肠溶胶囊。

Objective To observe the clinical efficacy of Xiaoyou Recipe on recurrent condyloma acuminatum（RCA） and its effect on the expression of Toll-like receptor（TLR） 1, TLR3 in peripheral blood mononuclear cells in CA patients. Methods Sixty qualified RCA patients were randomized into treatment group and control group, 30 cases in each group. All the patients received microwave treatment for removing the visible warts.Additionally, treatment group was given Xiaoyou Recipe orally and external use of Chinese medicine for the affected region, while the control group took oral use of thymosin enteric-coated capsule and external use of Shulening for the affected region. The treatment covered one month, and the follow-up lasted for 6 months.Before treatment and 90 days after treatment, the rash relapse and recurrence rate were recorded, and the expression levels of TLR1,TLR3 in peripheral blood mononuclear cells were determined by immunofluorescent two-color flow cytometry. Results（ 1） The recurrence rate of the treatment group（ 83.3%） was significantly higher than that of control group（56.7%）, there being significant difference between the two groups（P〈0.05）.The recurrence rate of the treatment group（16.7%） was lower than that of the control group（ 43.3%）, and there was significant difference between the two groups（P〈0.05）.（2） Before treatment, the differences of the positive expression rates of TLR1 and TLR3 were insignificant in both groups（P〉0.05）. After treatment, the expression positive rates of TLR1 and TLR3 were increased in both groups（ P〈0. 05） compared with those before treatment, but the differences were insignificant between the two groups（P〉0.05）. Conclusion Both Xiaoyou Recipe and thymosin enteric-coated capsules can reduce the recurrence rate of RCA probably through promoting TLR1 and TLR3 expression, stimulating body immune response and breaking up the HPV virus immune escape mechanisms. However, Xiaoyou Recipe shows better therapeutic efficacy for RCA than thymosin enteric-coated capsules.
