Dynesys动态内固定与融合术治疗腰椎退变疾病的对比研究 被引量:8

摘要 目的:对比分析Dynesys动态内固定与腰椎后路椎间融合术(PLIF)治疗腰椎退变疾病临床治疗效果及影像学结果。方法:收集2009年1月至2013年4月腰椎退变患者80例,根据随机数字表法分为Dynesys组(Dynesys动态内固定治疗)和PLIF组(PLIF治疗),每组各40例,对比两组临床疗效和影像学特征。结果:Dynesys组手术的平均时间为(102.3±19.23)min,短于PLIF组的(128.4±21.87)min,两组比较有统计学差异(P<0.05);Dynesys组术中出血量为(227.1±43.2)m L,明显少于PLIF组的(361.2±67.3)m L,差异有统计学意义(P<0.05);两组患者术前ODI以及VAS评分差异均无统计学差异(P>0.05),术后3个月及末次随访ODI以及VAS评分较术前均有明显改善,差异均有统计学差异(均P<0.05),手术末次随访两指标间差异无统计学意义(P>0.05)。两组手术近侧邻近节段椎间高度无明显变化,两组术后末次随访节段椎间高度Dynesys组变化更明显(P<0.05);Dynesys组手术节段ROM由术前为7.1度下降到末次随访时的4.6度,而PLIF组则由术前为7.2度降至末次的0度(P<0.05)。Dynesys组5例患者出现影像学中邻近节段退变,PLIF组出现影像学中邻近节段退变16例,两组比较差有统计学差异(P<0.05),PLIF组出现1例邻近节段退变,行二次手术。结论:Dynesys动态内固定与腰椎后路椎间融合术均能取的理想的效果,与融合术相比,Dynesys动态内固定对保留手术阶段部分的ROM、邻近节段椎间ROM的增幅以及邻近节段退变的发生数率均较低。
