肝局灶结节性增生与肝细胞肝癌磁共振平扫及强化特征分析

Analysis of features of hepatic focal nodular hyperplasia and hepatocellular carcinoma with MRI scan and enhanced MRI

目的探讨肝癌(Ca)和肝局灶结节性增生(FNH)的MRI平扫及强化特征。方法选取30例肝Ca患者及30例FNH患者均接受MR平扫及增强扫描,比较肝Ca与FNH大小及在肝内的分布情况,分析和比较两者影像学特征,包括T1WI、T2WI信号特征,动脉期、门脉期及延迟期信号变化,中心瘢痕、供血动脉、假包膜、门静脉癌栓及肝硬化背景情况等。结果 1FNH位于肝包膜下的比例为40.0%,显著高于肝Ca的13.3%,差异有统计学意义(P<0.05)。2FNH与肝Ca的T1WI均以低信号为主,比例分别为83.3%和86.7%;T2WI均以高信号为主,比例均为86.7%。动脉期均表现为高信号(100.0%);门脉期肝Ca以低信号为主(93.3%),FNH以高信号为主(100.0%);延迟期肝Ca以低信号为主(90.0%),FNH以等信号为主(90.0%)。3FNH存在中心瘢痕和显示供血动脉的比例分别为83.3%和43.3%,均显著高于肝Ca;肝Ca存在假包膜、门静脉癌栓及肝硬化背景的比例分别为45.7%、30.0%和56.7%,显著高于FNH,差异有统计学意义(均P<0.05)。结论 MRI可以显示肝Ca和FNH的特征,在其诊断和鉴别诊断中具有较高的应用价值。

Objective To investigate features of hepatic focal nodular hyperplasia and hepatocellular carcinoma with MRI scan and enhanced MRI.Methods 30 liver Ca and FNH patients underwent unenhanced and enhanced MR scan.T1 WI,T2WI signal characteristics;arterial,portal,central scar,feeding arteries,fake capsule,portal vein thrombosis,cirrhosis background and delayed phase signal changes were observed.Results FNH located under the liver capsule proportion（40.0%）was significantly higher than that of liver Ca（13.3%）（P〈0.05）.FNH and liver of Ca were mainly.The rates of low signal T1 WI of FNH and liver of Ca were 83.3% and 86.7%.T2 WI high signal are based,both the proportion was The rates of T2 WI high signal of FNH and liver of Ca were 83.3% and 86.7%86.7%.Display of FNH central scar and artery were 83.3% and 43.3%,which were significantly higher than that of liver Ca.The pseudocapsule proportion（45.7%）,portal vein thrombosis（30.0%） and cirrhosis backgrounds（56.7%）of liver Ca were significantly higher than that of FNH（P〈0.05）.Conclusion MRI can show the characteristics of liver Ca and FNH,which has a higher value in the diagnosis and differential diagnosis.
