益气扶元颗粒治疗III度以下精索静脉曲张不育症45例临床观察

Effect of Yiqi Fuyuangranule treatment on infertility patients with varicocele-below three degrees

目的:观察益气扶元颗粒治疗III度以下精索静脉曲张性不育症患者的临床疗效。方法:将88例III度以下精索静脉曲张性不育症患者随机分为治疗组45例和对照组43例,治疗组给予益气扶元颗粒(免煎颗粒),2次/d,每次1袋,开水冲服;对照组给予左卡尼汀口服液(瓶),2次/d,每次2瓶,口服;两组共治疗12周治疗结束后随访24周。比较两组患者的临床疗效,精液质量[包括精液量、精子密度、精子活率、前向运动精子(PR)及精子畸形率],测定治疗前后左侧精索静脉管径的变化,随访观察患者配偶受孕情况,结果:两组治愈率(治疗组6.67%,对照组6.97%)比较,差异无统计学意义(P>0.05);治疗组显效率(55.56%)、有效率(24.44%)、总有效率(86.66%)与对照组(分别为41.86%、20.90%、76.74%)比较,差异有统计学意义(P<0.05),治疗组优于对照组。两组患者治疗后精液量、精子密度、精子活率、PR比治疗前提高,精子畸形率均比治疗前降低(P<0.05);并且治疗后治疗组精子密度、PR均高于对照组(P<0.05)两组治疗后亚临床型、I度、II度左侧精索静脉管径,治疗组比治疗前缩窄(P<0.05),对照组(P>0.05);治疗24周后随访,患者配偶共有6例确诊怀孕,其中治疗组3例,对照组3例。结论:益气扶元颗粒能够提高三度以下精索静脉曲张性不育患者的精液质量,缩窄亚临床型、I度、II度精索静脉管径。

Objectives： To observe the therapeutic effect of Yiqi Fuyuan Granule treatment on infertility patients with Varicocele-below three degrees. Methods： 88 infertility patients with Varicocele-below three degrees were divided into the treatment group and control group randomly,with 45 in treatment group and 43 in control group.The treatment group was given Yiqi Fuyuan granular treatment,2 times a day. The control group was given levocarnitine oral solution,2 times a day. The treatment course lasted for 12 weeks,followed by 24 weeks follow-up. The therapeutic effect and semen quality was observed after treatment. The diameter of left spermatic vein was measured by color doppler ultrasound before and after treatment. Results： The cure rates of the treatment group and control group were 6. 67% and 6. 97% respectively,with no significant difference（ P〉0. 05）. The markedly effective rate,effective rate and total effective rate was 55. 56%,24. 44% and 86. 66% respectively,which was superior to 41. 86%,20. 90% and 76. 74% in the control group,with statistically significant difference（ P〈0. 05）. The semen volume,sperm concentration,sperm motility and progressive motility（ PR） was significantly increased and sperm abnormality rate was significantly reduced in both groups after treatment（ P〈0. 05）. The diameter of left spermatic vein was narrowed after treatment in patients with subclinical,1 and 2 degree varicocele in the treatment groups（ P〈0. 05） but the control group（ P〉0. 05）. The patients were followed up for 24 weeks after treatment and there were 6 patients（ 3patients in the treatment group and 3 in the control group） whose sexual partners got pregnant. Conclusion： Yiqi Fuyuan granule treatment can improve the semen quality in infertility patients with varicocele-below three degrees and narrow the diameter of spermatic vein in patients with subclinical,1 and 2 degree varicocele.
