运用FOCUS-PDCA程序降低妇科肿瘤化疗住院患者跌倒发生率的效果观察

Effect of FOCUS-PDCA procedures on reduction of fall incidence in inpatients with gynecological tumors undergoing chemotherapy

目的观察FOCUS-PDCA程序降低妇科肿瘤化疗患者跌倒发生率的效果。方法运用FOCUS-PDCA程序分析妇科肿瘤化疗患者跌倒发生的原因,修订跌倒风险评估流程,改变排班模式,病区添置相关防跌倒用具和增加健康教育方式等。结果 2014年1-9月妇科肿瘤病房患者跌倒发生率明显低于2013年1-12月（P〈0.05）;入院后第二天,患者对防跌倒的各项知识认识与2013年相比无明显变化（P〉0.05）;化疗后第一天患者防跌倒知识知晓常识和意识明显升高（P〈0.05）,患者对跌倒后果的认识前后无明显变化（P〉0.05）;出院前,患者防跌倒各项知识均有了明显提高（P〈0.05）。结论运用FOCUS-PDCA程序有利于病房找到管理薄弱环节的关键点,提高工作质量,降低跌倒发生率。

Objective To investigate the effect of FOCUS-PDCA procedures on reduction of fall incidence in patients with gynecologic tumors undergoing chemotherapy. Methods The FOCUS-PDCA procedures were used to analyze the causes of fall. Then,we revised the assessment process of fall risk,changed the scheduling model,and added the fall prevention appliances in the wards as well as increased the health education measures. Results The fall incidence during January to September,2014 was significantly reduced when compared to the whole year of 2013（ P〈0. 05）. Although the fall prevention knowledge of patients on the second day in the hospital had no changes when compared to 2013,it was significantly improved on the first day after chemotherapy（ P〈0. 05）. Moreover,the knowledge and awareness were obviously improved when patients discharged the hospital（ P〈0. 05）. Conclusion Application of the FOCUS-PDCA procedures can find the weak link of the ward management,improve the quality of work,and reduce the fall incidence.
