Discover Medical Care Mode by Pharmacy Base on Evidence Principle with Examples

目的:探讨通过药学服务模式改变现行药品消费模式,促进合理用药的可行性。方法:依据临床用药指南,药师参与高血压、糖尿病患者全程用药管理,建立患者药学服务档案,制定药学服务计划。结果:高血压、糖尿病患者得到更好的药学服务,患者生活质量得到提高。结论:药师依据用药指南等最佳证据,转变其传统工作模式为药学服务模式,减少了药品资源的浪费。

OBJECTIVE ：To change the drug consumption pattern by means of pharmaceutical care and to promote rational use of drugs. METHODS： Based on medication guidance, clinical pharmacists were involved in the whole medication process of patients with hypertension and those with diabetes by developing pharmaceutical care file and pharmaceutical care plan. RESULTS： Better pharmaceutical care was available for the patients with hypertension and those with diabetes and the quality of life of these patients has been improved. CONCLUSIONS： Pharmacists transformed their traditional mode of pharmaceutical care by using the best evidences including medication guidance as references resulted in the reduction in drug resources.
