摘要
目的探讨手部肌腱损伤后的早期康复护理。方法针对2010-01~2014-05间在我院外科接受治疗的80例手部肌腱损伤患者进行康复训练。术后2 d,如患者伤口无渗血情况出现,可以进行适当的红外线照射,每天进行分指、并指活动,2次/d,2 min/次。拆线后进行手部腕关节松动训练,2次/d,10 min/次。为了避免肌腱与组织的黏连可以在松动训练之后使用中药熏洗和蜡疗来预防和缓解粘连。术后21 d时,每天进行患指运动5次,5 min/次,配合手部肌肉和关节的按摩,辅助理疗和超短波疗法。术后42 d时主动活动受伤关节,10次/d,间隔2 h。结果 80例手部肌腱损伤患者,达到TAM优良的兵力45例,占比56.25%;达到TAM良好的患者33例,达到41.25%;效果不佳患者2例占比2.50%。手部肌腱康复总有效率达到百分之97.50%。TAM优良及良好的78患者,手部肌腱没有发生粘连现象,灵活性良好,已经恢复正常的生活能力。剩余2例TAM效果差的患者出现黏连现象需要进行进一步的康复治疗,在进行粘连松解术后4周恢复良好。结论手部肌腱损伤的早期康复治疗有助于手部功能恢复。
Objective To explore the early rehabilitation nursing after hand tendon injuries. Methods 80 cases with hand tendon injury had rehabilitation training from January 2010 to May 2010 in the hospital. 2 days after operation,patients without bleeding wound had proper infrared irradiation,had fingers separation and combination twice a day,and had loose hand wrist joint training twice a day,ten minutes each time. In order to prevent tendon adhesion to organization,traditional Chinese medicine fumigation and kerotherapy can be used to prevent and alleviate adhesion. 21 days after surgery,patients had wounded finger exercise five times a day,5 minutes every time,with hand massage in the muscles and joints,auxiliary physiotherapy and ultrashort wave therapy. 42 d after the injured joint active activities,they had exercise 10 times a day,2 hours as the interval. Results In 80 cases with hand tendon injuries,45 cases had TAM excellence,accounting for 56. 25%; 33 cases good at TAM,accounting for 41. 25%; 2 ineffective cases accounted for 2. 50%. Tendon healing hands total effective rate reached 97. 50%. 78 cases had excellent and good TAM. Hand tendon adhesion phenomenon did not occur,flexibility was good,and returned to normal life. The remaining 2 cases who had poor adhesion phenomenon needed further rehabilitation,and after 4weeks' rehabilitation,they recovered. Conclusion Hand tendon injury early rehabilitation therapy can help the hand function recovery.
