摘要
碎裂QRS波(fragmented QRS complex,fQ RS)是指在排除束支阻滞基础上,心电图相邻2个以上导联出现的一个以上的R波,或R波、S波切迹,,是一项简单而又无创的心电学指标。研究证实fQ RS波与心肌瘢痕或心肌纤维化导致的心肌电活动异常相关。fQ RS波可见于多种心脏疾病,包括冠心病、缺血性和非缺血性心肌病、Brugada综合症、长QT综合症、致心律失常性右室心肌病、室壁瘤等,且可能与疾病预后相关。在此,本文对fQ RS波的定义、发生机制及其临床意义进行了阐述。
Fragmented QRS complex（fQRS） is defined as presence of〉 1 R wave in two contiguous ECG leads, or notching in the nadir of the R wave or the S wave. It is an easily operated and noninvasive way for electrocardiological evaluation. Previous study reported that this index related to the abnormality of cardiac electrical activity due to myocardial scar or fibrosis in various cardiovasular diseases, including coronary artery disease, ischemic and non-ischemic cardiomyopathy, Brugada syndrome, long QT syndrome, arrhythmogenic right ventricular cardiomyopathy and ventricular aneurysm, and may predict the prognosis. This review elaborated the definition, mechanism and clinical significance of fQRS.
