我院静脉用药调配中心不合理医嘱分析

Analysis of irrational medical order in pharmacy intravenous admixture service of our hospital

目的通过药师审方,发现并分析我院静脉药物配置中心的不合理用药,促进临床合理用药。方法对我院近一年静脉药物配置室审方结果进行分类统计与分析。结果发现不合理用药医嘱866张,占调配总量的0.098%,包括溶媒、电脑录入失误、给药浓度、给药频次、配伍、给药剂量等不当的问题。经反馈给临床,98.5%的不合理用药医嘱得以纠正。结论静脉药物配置中心药师审核医嘱可提高静脉用药的合理性、安全性和有效性。

Objective To discover and analyze the irrational prescriptions of pharmacy intravenous admixture service of our hospital and to promote clinical rational drug use. Methods The results of the prescription audit of pharmacy intravenous admixture service in the past one year were statistically classified and analyzed. Results 866 cases of irrational prescription medications were found, accounting for 0.098%, including computer entry errors, improper solvent selection, drug concentration, frequency of administration, incompatibility, dose, etc. Through the feedback to the clinical departments, 98.5% of irrational medication orders were corrected. Conclusion Through the pharmacist＇s prescription audit, the rationality, safety and efficacy are improved in the intravenous drug use.
