以“位置性眩晕”为首发症状的肺癌脑转移1例分析及文献复习

“Positional Vertigo” as the first sign of brain metastases in patients with lung cancer and literature review

脑是各种恶性肿瘤常见的转移部位,肺癌脑转移瘤约占全部颅内转移瘤的18%~45%,为最常见的脑转移瘤。肺癌早期常缺乏呼吸系统表现,伴脑转移后临床表现也不典型,如头痛、呕吐以及局部神经定位症状如偏瘫、失语、癫痫等,在临床中容易误诊和漏诊。位置性眩晕是神经科较为常见的症状,其最常见病因为＂良性阵发性位置性眩晕（BPPV）＂。现报道以＂位置性眩晕＂为首发症状的肺癌脑转移1例,并结合文献复习,以提高对该病的认识。

Brain metastases from lung cancer is the most common type and account for about 18% to 45% in all intracranial metastase tumors. Patients with lung cancer often lack respiratory system performance in early period of the disease.Clinical manifestations of brain metastasis are not typical, such as headache,vomiting and local nerve symptoms like hemiplegia,aphasia,epilepsy,etc,and it is easy to cause misdiagnosis and therapeutic error. Vertigo is a common neurological manifestation, the common cause is BPPV.We report a case of Positional Vertigo as the first sign of brain metastases from lung cancer. A systematic review of the literature on the pathogenesis and treatment of lung cancer with brain metastasis is included.
