北京市乡村两级卫生机构医患信任影响因素调查

Investigation on the Influencing Factors of Doctor- patient Trust in Rural Beijing

目的:了解北京市乡村两级医疗卫生机构医患信任现状,探索有利于增加乡村两级医患信任的因素。方法:采用分层随机抽样的方法,在北京市H区抽取12个行政村,运用参与式与非参与式观察相结合的方法进行实地观察。结果:两级医疗卫生机构比较,除村干部与村民外,其他各类调查对象均对乡镇卫生院医患信任评价较高(多数在3.8分以上);村干部及村民对村卫生室的医患信任评价较高,均达到4分及以上。在现有的诊疗条件中,有利于促进乡村两级医疗卫生机构医患信任的因素主要有医疗费用、诊疗技术、服务态度和医患熟识关系。结论:乡村两级医疗卫生机构可通过改善服务态度,提高医疗技术水平,与患者建立熟悉关系等方法来增进医患信任。

Objective： To learn the current situation of healthcare organizations doctor- patient trust in rural areas in Beijing,and to explore influencing factors beneficial to doctor- patient trust in rural areas. Methods： Stratified random sampling method was used to select 12 administrative villages in H district in Beijing. Use participatory and non- participatory observation method to grasp the current situation of doctor- patient trust. Results： Compared the rural healthcare organizations,all the participants but villagers＇ managers and villagers gave higher scores to doctor- patient trust of large hospitals. The scores were almost higher than 3. 8. By contrast,villagers＇ managers and villagers gave higher scores to doctor- patient trust of village clinics（ higher than 4）. Among available medical conditions,medical expense,treatment technique,service attitude and familiarity were good for improving doctor- patient trust in rural areas. Conclusion： Healthcare organizations in rural areas may enhance doctor- patient trust by improving service attitude,powering medical technique,familiarized with patients and so on.
