解毒化瘀颗粒对慢性重型肝炎患者相继器官衰竭估计评分数值的影响 被引量:3

Effect of detoxification and stasis-resolving granules on sequential organ failure assessment scores in patients with chronic severe hepatitis

摘要 目的:明确解毒化瘀颗粒对慢性重型肝炎(CSH)患者相继器官衰竭估计(SOFA)评分、生化指标、内毒素水平的影响。方法:选择符合诊断标准的CSH患者110例,按照治疗前SOFA评分数值,随机分为治疗组55例和对照组55例。两组患者均采用治疗原则及治疗措施相同的西药综合治疗方法;治疗组患者在此基础上,加服解毒化瘀颗粒。记录两组患者治疗过程中各个观察时点的SOFA评分数值、生化和血清内毒素水平等。结果:治疗期间,治疗组患者最大的SOFA评分明显低于对照组的最大值(P<0.05),且治疗后存活患者SOFA评分为6.23±2.87,而死亡患者为9.53±2.86(P<0.001);治疗组患者生化和内毒素指标的改善均明显优于对照组(P<0.05)。结论:加用解毒化瘀颗粒能明显降低SOFA评分,且能更好地改善患者肝功能、缩短凝血时间、降低内毒素水平,具有较好的临床疗效。 Objective: To investigate the effects of the detoxification and stasis-resolving granules on the sequential organ failure assessment( SOFA) scores,biochemical parameters and endotoxin levels in patients with chronic severe hepatitis( CSH). Methods: One hundred and ten patients who met the diagnostic criteria of CSH were included. Based on their pretatment SOFA scores,the patients were equally randomized to receive either comprehensive western treatment alone( n = 55,control group) or in combination with the oral detoxification and stasis-resolving granules( n = 55,trial group). SOFA scores,biochemical parameters and serum endotoxin levels were recorded for patients in both groups at each observation point during treatment. Results: During treatment,the highest SOFA score indentified in the trial group was significanty lower than that in the control group( P 0. 05). SOFA score was 6. 23 ± 2. 87 and 9. 53 ± 2. 86 in patients who survived or died after treatment,respectively( P 0. 001). Conclusions: The detoxification and stasis-resolving granules were associated with a significantly lower SOFA score,improved liver function,shortened coagulation time,and decreased endotoxin in patients with CSH.
