SPECT/CT脑血流灌注显像两种半定量分析法诊断缺血性脑病价值比较 被引量:7

摘要 目的:比较SPECT/CT脑血流灌注显像两种半定量分析方法诊断缺血性脑病的价值。方法:选择无神经系统疾病15例为对照组,行SPECT/CT脑血流灌注显像检查,按照公式x-1.645s计算各部位与小脑比值下限值。选择缺血性脑病61例为观察组,在发病7天内行SPECT/CT脑血流灌注显像检查,由3名具备5年以上工作经验的核医学科医师单独阅片进行定性判断;然后分别采用对侧比值法和小脑比值法进行半定量分析,比较两种半定量分析方法对单侧病变和双侧病变的检出情况。结果:观察组61例中,定性判断缺血性病灶90个,其中单侧病变56个,双侧病变34个。对侧比值法检出单侧病变病灶55个,检出率98.2%,显著高于小脑比值法的80.4%(P<0.05);双侧比值法检出双侧病变病灶14个,检出率41.2%,非常显著低于小脑比值法的91.2%(P<0.01)。结论:SPECT/CT脑血流灌注显像的两种半定量分析方法各有优缺点,对于单侧病变的缺血性脑病推荐选择对侧比值法,而双侧病变则建议采用小脑比值法。
