白内障手术角巩膜缘切口和透明角膜缘切口的疗效比较

Comparison of Curative Effect of Cataract Operation Through Corneoscleral Limbus Versus Clear Corneal Incisions

目的比较角巩膜缘切口和透明角膜缘切口白内障超声乳化吸除、人工晶体植入术后的疗效。方法选取本院2011年1月至2014年1月收治的120例白内障患者，所有患者均行超声乳化、人工晶体植入术。将患者随机分为观察组与对照组，每组各60例。观察组采用角巩膜缘切口，对照组采用透明角膜缘切口，观察两组患者治疗后d1、术后1周、1个月、3个月的视力、散光程度以及并发症情况。结果观察组术后各时间段裸眼视力显著优于对照组，角膜散光程度也显著小于对照组，差异均有统计学意义（均 P ＜0．05）；两组患者在并发症发生情况上无显著性差异（ P ＞0．05）。结论对白内障患者实施超声乳化吸除、人工晶体植入术，使用改良角巩膜缘切口超声乳化吸除的效果显著优于透明角膜缘切口，患者术后视力恢复情况好，角膜散光程度较小，值得临床推广应用。

[Objective]To explore the curative effect of corneoscleral limbus versus clear corneal incisions during phacoemulsification and intraocular lens implantation .[Methods]A total of 120 cataract patients were selected from January 2011 to January 2014 to undergo phacoemulsification and intraocular lens implantation . They were divided into observation and control groups （ n=60 each） .The observation group underwent lim‐bal incision while the control group had clear corneal incision .Both groups were observed with regards to vi‐sion ,astigmatism and complications at Day 1 ,Week 1 ,Month 1 and Month 3 .[Results]The parameters of un‐corrected visual acuity and astigmatism were much better in observation group than those in control group . And the differences were statistically significant （ P 〈0 .05） .But no significant inter‐group difference existed in complications （ P〉 0 .05 ） .[Conclusion]During phacoemulsification and intraocular lens implantation for cataract patients ,use of modified corneoscleral rim incision is significantly superior to that of clear corneal inci‐sion .The postoperative visual acuity is better with a smaller degree of corneal astigmatism .And it is worthy of wider clinical applications .
